Research On The Influencing Factors Of HIV Self-Testing Behavior In Men Who Have Sex With Men

Objective To analyze the HIV self-testing status and possible influencing factors of the MSM population in the three cities of Suzhou,Wuhan and Chengdu,in order to grasp the behavioral characteristics of this population and to promote their HIV self-testingMethods This study was conducted in Suzhou,Wuhan and Chengdu during June to December 2022.Eligible participants were recruited through nongovernmental organization(NGO)on-site recruitment,online recruitment and MSM peer recommendation.The questionnaire survey was completed online by scanning the code by mobile phone.The contents of the questionnaire mainly included demographic characteristics,HIV testing and HIV self testing experience,male sexual partner and sexual behavior information,internalized homophobia of participants,mental health,awareness/experience and self-efficacy of Pr EP/PEP.Describe the basic information of the participants;univariate analysis were performed byχ~2test and t test,etc;and multivariate analyses using non-conditional logistic regression model.All used two-sided tests with test levelα=0.05.Results A total of 1554 eligible MSM were included in this study.The age of the participants was between 18 and 70 years,with an average age of 30.7±8.79 years.96.7%had undergone HIV testing,77.1%were unmarried and had no girlfriends,50.9%had no male partner,97.7%identified themselves as male,80.7%considered their sexual orientation homosexual,97.3%were aware of the HIV self-testing method,86.7%had previously undergone HIV self-testing,88.4%had 1 to 5 male same-sex partners,11.6%had 6 to 10 or more same-sex partners,67.5%had first anal intercourse between 18 and 24 years,85.9%used gay male dating apps(Blued,Aloha,etc.),56.2%had depression,43.2%had anxiety disorders,82.2%had heard of Pr EP,88.8%had heard of the PEP.Multivariate analyses affecting HIV self-testing showed:age 18～24 years(OR:3.234,95%CI:1.748～5.984),From 25 to 29 years old(OR:3.009,95%CI:1.641～5.518),From 30 to 39 years old(OR:2.626,95%CI:1.464～4.710),Education degree of junior college(OR:2.010,95%CI:1.130～3.575),Bachelor’s degree or above(OR:2.484,95%CI:1.421～4.339),The survey city is Wuhan(OR:3.567,95%CI:2.465～5.163),and the survey city is Chengdu(OR:2.233,95%CI:1.513～3.296),With a male partner but not living together(OR:1.749,95%CI:1.209～2.529),Disclosed his sexual orientation to his classmates/colleagues(OR:1.682,95%CI:1.186～2.386),The frequency of condom use during sex with men in the last 6 months was sometimes used(OR:1.597,95%CI:1.021～2.498),Fixed and incidental partners in the last 6 months(OR:1.757,95%CI:1.211～2.546),Fixed,incidental and commercial partners in the last 6 months(OR:1.725,95%CI:1.198～2.485),It is not clear if there was HIV positive in the last 6months(OR:1.503,95%CI:1.048～2.156),In the last 6 months,through gay dating software(Blued,Aloha,etc.)(OR:1.622,95%CI:1.113～2.363),Rush was used in the last 6 months(OR:1.922,95%CI:1.400～2.638),Know the Pr EP(OR:2.184,95%CI:1.561～3.055),Event-driven protocol using Pr EP in the last 6 months(OR:3.336,95%CI:1.444～7.706),Daily regimen of Pr EP in the last 6 months(OR:4.622,95%CI:1.443～14.807),Know the PEP(OR:2.047,95%CI:1.387～3.022)were all positively correlated with HIV self-test;Age at first anal intercourse was from 25 to 29years(OR:0.499,95%CI:0.264～0.943)and at least 30 years old(OR:0.319,95%CI:0.163～0.623)were negatively correlated with HIV self-testing.Conclusions(1)The MSM who have done HIV self-testing in Suzhou,Wuhan and Chengdu in the last 6 months are relatively young and have a high educational level.Most of them were unmarried and without male partners,sexual orientation is mostly gay,and the vast majority of MSM self-identified with the gender as male.(2)The MSM population has a high rate of HIV self-testing,but it is still far from the first 95%target proposed by the World Health Organization.We can in the traditional offline HIV prevention work(such as HIV on-site test,HIV confirmatory testing,condom distribution)at the same time,actively explore online HIV interventions to expand coverage,such as,through MSM dating software(Blued and Aloha software,etc.)or We Chat platform online HIV prevention,HIV self testing methods,AIDS treatment and care and Pr EP/PEP knowledge and information.So as to improve the HIV self-testing and HIV test rate of MSM population,reduce the HIV infection rate in MSM.
