Objective: To investigate the status of hospitalized middle-aged and elderly patients with sarcopenia,and to analyze its clinical characteristics and related risk factors.To help clinical early identification of sarcopenia and provide a basis for the prevention and treatment of sarcopenia in middle-aged and elderly hospitalized patients.Methods and subjects: In this cross-sectional study,a total of 125 middle-aged and elderly patients over 50 years old with sarcopenia who received medical treatment from the Endocrinology Department of Sichuan Academy of Medical Sciences and Sichuan Provincial People’s Hospital from June 2020 to November 2022 were chosen as the case study group,while 173 non-sarcopenia elderly people hospitalized during the same period were chosen as the control group by a cross-sectional study.General information and clinical data were collected,with muscle mass,muscle strength,and muscle function of all subjects being measured by dual-energy X-ray absorptiometry,grip strength measurement,and calculation of walking speed.Laboratory biochemical indicators were also detected,and statistical analysis was performed by SPSS26.0software.Result:(1)Of the 298 middle-aged and elderly hospitalized patients included in this study,the prevalence rate of sarcopenia accounted for 41.94%.(2)Compared with the non-sarcopenia group,the sarcopenia group had lower BMI,systolic blood pressure,mid-arm circumference,calf circumference,gait speed,parents’ history of fragility fracture,history of a fragility fracture after 50 years old,calcium supplement,vitamin D supplement,and the proportion of taking anti-osteoporosis drugs.The proportions of smoking,alcohol drinking,falls,coffee/strong tea drinking,and polypharmacy were higher in the sarcopenia group.There were no significant differences between the two groups in age,age at menopause,diastolic blood pressure,grip strength,pain score,comorbidities,education level,and height loss.(3)Comparison of biochemical indexes between the sarcopenia and non-sarcopenia groups: total protein and albumin in the sarcopenia group were lower;CRP was higher.There were no significant differences in Hb,TC,TG,LDL-C,HDL-C,AST,ALT,TBIL,DBIL,BUN,Cr,e GFR,and Hb A1 c levels between the two groups.(4)Comparison of hormones between sarcopenia and non-sarcopenia groups: in males,LH was higher in the sarcopenia group,and there were no significant differences in E2,Prog,Testo,PRL,and FSH levels between the two groups.In females,the levels of LH and FSH were higher in the sarcopenia group,and there were no significant differences in the levels of E2,Prog,Testo,and PRL between the two groups.There were no significant differences in the levels of TSH,FT3,FT4,ACTH,8am Cor,and IGF-1 between the two groups.(5)Comparison of serum bone metabolism markers between sarcopenia and non-sarcopenia group: the sarcopenia group had lower serum calcium and higher P1 NP.There were no significant differences in serum phosphorus,ALP,β-CTX,and 25(OH)D levels between the two groups.(6)Comparison of BMD between sarcopenia and non-sarcopenia groups: there was no significant difference in BMD levels of the femoral neck,greater trochanter,total hip,and L1-4 area between the two groups.(7)With the occurrence of sarcopenia as the dependent variable and the statistically significant difference between the two groups as the independent variable,multiple logistic regression analysis showed that male and CRP increased risk factors for sarcopenia,BMI and higher calf circumference were protective factors for sarcopenia.Conclusion: The prevalence of sarcopenia is high in middle-aged and elderly inpatients in the department of endocrinology.Male and increased CRP are independent risk factors for sarcopenia,while increased BMI and calf circumference are protective factors for sarcopenia.
