| Objective:By observing the clinical efficacy of Gui Zhi Fu Ling Wan plus flavor combined with fenmodong in the treatment of blood stasis type of perimenopause in patients, and comparing its efficacy difference with that of fenmodong,we provide new clinical treatment ideas for the treatment of perimenopausal syndrome. Methods:In this study,66 patients with blood stasis type perimenopausal syndrome were collected in strict accordance with the nadir criteria, and were divided into two groups of 33 patients each using the random number table method, with the control group being treated with 1/10 mg of fenmodone and the treatment group being treated with Gui Zhi Fu Ling Wan plus flavor orally every 28 days for 3 cycles. The changes of Kupperman score, total score, efficacy of individual TCM symptoms, serum sex hormone levels(follicle stimulating hormone, luteinizing hormone, estradiol)and comprehensive clinical efficacy were observed and recorded in the two groups. The difference was considered statistically significant at P<0.05.Results:1.Comparison of general data: The differences were not statistically significant(P>0.05)and were comparable between the two groups in terms of age, disease duration, menstrual status and the observed indexes before treatment.2.Comparison of Kupperman scores: Both treatment regimens reduced Kupperman scores, and Kupperman scores in the treatment group were significantly lower than those in the control group, with statistically significant differences(P<0.05).3.Comparison of the total TCM evidence points: both treatment regimens reduced the total TCM evidence points, and the total TCM evidence points in the treatment group were significantly lower than those in the control group, with statistically significant differences(P<0.05).4.Comparison of the TCM syndrome scores: After treatment, all the TCM syndrome scores of patients in both groups decreased compared with that before(P<0.05).Compared with the control group, the differences were statistically significant(P<0.05)in improving the symptoms of hot flashes and sweating, tingling, menstrual disorders(dysmenorrhea or blood clots),skin and nail disorders, chest distension, and irritability in the treatment group, and not statistically significant(P>0.05)in the symptoms of numbness of limbs and insomnia and dreaminess.5.Comparison of serum sex hormone levels: After treatment, estradiol levels increased and follicle-stimulating hormone and luteinizing hormone levels decreased in both groups compared with those before, with statistically significant differences(P<0.05).After treatment, estradiol levels were better regulated in the treatment group than in the control group, with statistically significant differences(P<0.05);follicle-stimulating hormone and luteinizing hormone levels were not statistically significant when compared between the two groups(P>0.05).This indicates that the two treatment regimens were comparable in improving follicle-stimulating hormone and luteinizing hormone levels.6.Comparison of comprehensive clinical efficacy and long-term clinical efficacy: the total effective rate of the treatment group and the control group were 93.54% and 78.13%,respectively,with statistically significant differences(P<0.05).The TCM evidence points of the treatment group were significantly lower than those of the control group after 3 months of drug discontinuation, and the difference was statistically significant(P<0.05).This indicates that the long-term clinical efficacy of the treatment group was better. Conclusions:The combination of Gui Zhi Fu Ling Wan plus Wei and Fenmutong has definite clinical efficacy in the treatment of perimenopausal syndrome, which can effectively improve various clinical symptoms caused by "blood stasis",reduce the Chinese medicine syndrome score and Kupperman score of patients, and significantly improve the E2 level, but not the improvement of FSH and LH.The clinical efficacy and low recurrence rate after treatment are worth promoting as they have certain reference value for guiding clinical treatment. |
