Objective:Sanshi Shengxin ointment was applied to wound dressing change of patients after low simple anal fistula surgery,and the wound pain and secretions of patients before and after treatment were observed,as well as the levels of inflammatory factors IL-6 and TNF-α and the expression levels of growth factors VEGF and bFGF,etc.,so as to evaluate the efficacy of Sanshi Shengxin ointment on wound healing after low simple anal fistula surgery.To provide a reliable basis for clinical application.Methods:Seventy-two patients who met the inclusion criteria were randomly divided into the control group and the treatment group with 36 patients in each group.All patients in both groups underwent anal fistulotomy.Both groups were given routine treatment after surgery,and the treatment group was given potassium permanganate fumigation sitting bath on the basis of routine treatment,and Sanshi Shengxin paste gauze packing and dressing change.The control group was treated with potassium permanganate fumigation and sit-bath,and Vaseline yarn strip was used for dressing change.The differences in age,gender and original wound area between the two groups were compared.The postoperative wound pain,secretions and wound healing rate between the two groups were compared,as well as the expression levels of IL-6,TNF-α,VEGF and bFGF in wound secretions between the two groups,and the overall clinical efficacy of the two groups was evaluated.Result:1.Comparison of general information between the two groups:there was no statistical significance in age,gender and original wound area(P>0.05),indicating that the two groups of patients were comparable.2.Comparison of wound pain scores:(1)Intra-group comparison:the differences between the two groups were statistically significant on day 7 after surgery compared with day 2 after surgery,on day 14 after surgery compared with day 7 after surgery,and on day 21 after surgery compared with day 14 after surgery(P<0.05).(2)Comparison between groups:2d after surgery,there was no statistical significance between the two groups(P=0.573>0.05),and 7d,14d and 21d after surgery,there was statistical significance between the two groups(P<0.05).3.Comparison of wound secretion scores:(1)Intra-group comparison:7d after surgery compared with 2d after surgery,14d after surgery compared with 7d after surgery,21d after surgery compared with 14d after surgery,the differences were statistically significant(P<0.05).(2)Comparison between groups:2d after surgery,there was no statistical significance between the two groups(P=0.791>0.05),and 7d,14d and 21d after surgery,there was statistical significance between the two groups(P<0.05).4.Comparison of wound healing rate:(1)Intra-group comparison:compared with the postoperative day 14d and the postoperative day 7d,compared with the postoperative day 21d and the postoperative day 14d,the differences were statistically significant(P<0.05).(2)Comparison between groups:The difference between the two groups was statistically significant on the 7d,14d and 21d after surgery(P<0.05),and the difference was more significant on the 14d and 21d after surgery(P<0.001).5.Comparison of the levels of inflammatory cytokines IL-6 and TNF-α:(1)Comparison between groups:2d after surgery,there was no statistical significance in the levels of IL-6 and TNF-α in wound secretions between two groups(P>0.05);The levels of IL-6 and TNF-α in wound secretions were significantly different between the two groups on day 7 and day 14 after surgery(P<0.001).(2)Intra-group comparison:the difference between the two groups was statistically significant on day 7 after surgery compared with day 2 after surgery,and on day 14 after surgery compared with day 7 after surgery(P<0.05).6.Comparison of the levels of growth factor VEGF and bFGF:(1)Comparison between groups:There was no statistical significance in the levels of VEGF and bFGF in wound secretions of the two groups 2d after surgery(P>0.05);There were significant differences in the levels of VEGF and bFGF in wound secretions between the two groups on day 7 and day 14 after surgery(P<0.001).(2)Intra-group comparison:the difference between the two groups was statistically significant on day 7 after surgery compared with day 2 after surgery,and on day 14 after surgery compared with day 7 after surgery(P<0.05).7.Comparison of overall efficacy:After 21 days of treatment,the total effective rate of the control group was 94.4%,and the total effective rate of the treatment group was 100%,with statistical significance(P=0.022<0.05).Conclusion:Sanshi Shengxin ointment can reduce wound pain in patients with low simple anal fistula surgery,reduce wound secretion,reduce IL-6,TNF-α levels,inhibit wound inflammatory response,improve VEGF and bFGF expression levels,promote growth factor generation,improve wound healing rate,promote wound healing,clinical efficacy is significant,worthy of promotion and application.
