Objective: To observe the effect of combining short-term insulin intensive therapy with Yi Qi Qi Yao decoction on blood glucose fluctuation in patients with type 2 diabetes mellitus(T2DM)with Qi and Yin deficiency,and to explore its mechanism of action.Methods: Sixty patients with T2 DM with deficiency of both qi and yin who met the criteria of this study were selected and then randomly divided into two groups(30patients in the control group and 30 patients in the observation group).The control group was treated with insulin intensive therapy on top of diabetes education and other basic treatments;the observation group was treated with Yi Qi Qi Yao decoction on top of the control group,and both groups were constantly observed for 14 days(1 course of treatment).FBG、2h PG、GA、MBG、CV、SDBG、MAGE、TIR、TAR、Insulin dosage、C-P、HOMA-IR、TNF-α、IL-6、CRP、TCM syndrome score,and the safety indexes were tested in both groups before and after treatment.Results: 1.Comparison of blood glucose indexes: FBG,2h PG and GA decreased significantly in both groups after treatment compared with those before treatment(P<0.01),and the decrease was more obvious in the observation group than in the control group.(P<0.05).2.Comparison of blood glucose fluctuation indexes: MBG,CV,SDBG,MAGE and TAR in both groups decreased significantly after treatment compared with those before treatment(P<0.01),and the decrease was more obvious in the observation group than in the control group(P < 0.05);TIR in both groups increased significantly after treatment compared with that before treatment(P<0.01),and the increase was more obvious in the observation group than in the control group(P<0.05).3.Comparison of insulin dosage: After treatment,insulin dosage was significantly reduced in both groups compared with that before treatment(P < 0.01),and the reduction was more obvious in the observation group than in the control group(P <0.05),and the average daily insulin dosage was significantly lower in the observation group than in the control group during treatment in both groups(P<0.05).4.Comparison of islet function-related indexes: C-P and HOMA-IR decreased significantly in both groups after treatment compared with those before treatment(P<0.01),and the decrease was more obvious in the observation group than in the control group(P<0.05).5.Comparison of serum inflammatory factor indexes: Serum TNF-α,IL-6 and CRP in both groups decreased significantly after treatment compared with those before treatment(P<0.01),and the decrease was more obvious in the observation group than in the control group(P<0.05).6.Comparison of the TCM syndrome score:The TCM symptom scores of both groups decreased significantly after treatment compared with those before treatment(P<0.01),and the decrease was more significant in the observation group than in the control group(P<0.05).7.Comparison of the efficacy of the TCM symptom score: the total effective rate of the control group and the observation group were 90% and 100%,respectively,and there was a significant difference(P<0.05)between the two groups.8.Safety indicators : During the 14-day(1 course)treatment,the frequency of hypoglycemia and TBR in the observation group were significantly lower than those in the control group(P<0.05),and no abnormal liver or kidney function and no other adverse reactions were observed before and after treatment in both groups..Conclusion: Patients with T2 DM with Qi and Yin deficiency have excellent clinical efficacy under intensive treatment with Yi Qi Qi Yao decoction combined with short-term insulin,with significant advantages in lowering blood glucose levels,reducing blood glucose fluctuations and insulin dose,avoiding hypoglycemic events,and improving symptoms at the level of presumably involved in lowering serum TNF-α,IL-6,and CRP levels,improving the inflammatory response of the patient’s organism,reducing insulin resistance,and restored insulin sensitivity related.This regimen is safe,efficacious,without adverse side effects,and deserves further study.
