Objective:To observe the effect of auricular pressure pills on cardiac function and sleep quality of patients with chronic heart failure with insomnia(Qi and Yin deficiency evidence),evaluate its clinical efficacy, and provide experience for the treatment of chronic heart failure with insomnia with traditional Chinese medicine.Method:From December 2021 to December 2022,60 patients with insomnia(qi deficiency and yin deficiency)and chronic heart failure who met the inclusion criteria were selected from the outpatient department of cardiovascular medicine and inpatients of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine. Both groups of patients were given routine drug treatment and simple psychological counseling; In the treatment group, auricular pressure pillss were added on the basis of the control group, twice a week, and the left and right ears rotated. The course of treatment in both groups was 4 weeks. The TCM symptom score, left ventricular ejection fraction(LVEF),cardiac function grade(NYHA),Pittsburgh Sleep Quality Index(PSQI),Minnesota Heart Failure Survival Questionnaire(MLHFQ),and the serum level of n-terminal-b pro-brain natriuretic peptide(NT-pro BNP)before and after treatment were statistically analyzed by SPSS23.0.Results:(1)TCM symptom score: after treatment, the five symptoms of palpitation and weakness, disturbed and sleepless, spontaneous sweating and night sweating, shortness of breath and dry mouth and throat in the two groups were improved to varying degrees compared with those before treatment. There were statistically significant differences between the two groups in the symptoms of palpitation, fatigue, disturbed and sleepless, spontaneous sweating and night sweating, and shortness of breath(P<0.05);There was no significant difference in symptoms of dry mouth and throat between the two groups(P>0.05).The total effective rate of TCM symptoms in the treatment group and the control group was 86.67% and 63.33% respectively, with a statistically significant difference between the two groups(P<0.05).(2)Left ventricular ejection fraction: LVEF of the two groups after treatment was significantly higher than that before treatment(P<0.05);There was no significant difference between the two groups(P>0.05).(3)Cardiac function classification: after treatment, the cardiac function classification of patients in the two groups was improved compared with that before treatment, and the difference was statistically significant(P<0.05);Compared with the two groups, the cardiac function classification in the treatment group improved more significantly(P<0.05).(4)Pittsburgh Sleep Quality Index Scale: The PSQI score of the two groups after treatment was improved compared with that before treatment, and the difference was statistically significant(P<0.05);Compared with the two groups, the PSQI score of the treatment group improved significantly(P<0.05).(5)Minnesota living with heart failure questionnaire: The MLHFQ scores of patients in the two groups decreased after treatment, but the difference between the treatment group and the treatment group was statistically significant(P<0.05),while the difference between the control group and the treatment group was not statistically significant(P>0.05),and the MLHFQ scores in the treatment group decreased more significantly(P<0.05).(6)Serum N-terminal-B brain natriuretic peptide precursor levels: NT-pro BNP water in the two groups after treatment was significantly lower than that before treatment(P<0.05);Compared with the group, the level of NT-pro BNP in the treatment group decreased more significantly(P<0.05).(7)No adverse reactions occurred during the treatment in both groups, and the safety indicators were within the normal range. Conclusion:Auricular pressure pills can effectively improve the sleep quality of CHF patients, reduce TCM symptom score, improve clinical symptoms and improve the quality of life of patients;Reduce the serum NT-pro BNP level, increase the left ventricular ejection fraction, and improve the cardiac function of patients; It is safe and can be further promoted in clinical research.
