Objective Using Anderson’s model as a theoretical framework,we understand the current situation of the willingness of the elderly to seek Chinese medicine services in Hefei, explore the relevant factors affecting the willingness of the elderly to receive Chinese medicine services in Hefei, and provide a theoretical basis for improving the capacity of Chinese medicine services and better meeting the development of the elderly’s health service needs.MethodsA multi-stage random sampling method was used to select 575 cases of elderly people in the Hefei community from January 2022 to June 2022 for the questionnaire survey. The questionnaire was designed and categorised based on the Anderson model, including propensity factors, enablement factors and demand factors and satisfaction. The data were statistically analysed using descriptive statistics,chi-square test and dichotomous logistic regression using SPSS 26.0 software. Finally, the fishbone analysis was used to carry out a deeper analysis of the influencing factors.Results1.Among the 575 survey respondents,209 elderly people(36.3%)were not willing to prefer Chinese medicine primary care institutions, while 366 elderly people(63.7%)were willing to prefer Chinese medicine primary care institutions.2.Model 4 was the best in terms of overall goodness of fit, with an explanatory degree of 34.3%.Of the four dimensions, satisfaction has the greatest effect on the dependent variable, followed by the enabling factor, and the least effect is the demand factor.3.The results of the multi-factor analysis showed that, among the propensity factors, the degree of recognition of Chinese medicine was statistically different in the choice of the first visit to Chinese medicine services for the elderly.Among the enabling factors, the results showed that distance to medical care and knowledge of the family doctor contracting system were statistically different in the choice of the first visit to Chinese medicine services for the elderly.Among the need factors, the frequency of having chronic diseases and routine medical check-ups were not statistically significant. For patient satisfaction factors, there were also statistically significant differences in the choice of the first visit to Chinese medicine services for the elderly in terms of the attitude of the Chinese medicine practitioner, the effectiveness of the Chinese medicine visit, and the cost of the Chinese medicine visit.Conclusion1.Relatively high willingness of elderly people to seek first consultation for TCM services in Hefei. The degree of recognition of TCM is a key factor affecting the willingness of the elderly to seek first consultation for TCM services. Therefore,it is necessary to further strengthen the publicity and education of TCM,popularise TCM knowledge, enhance the health literacy of the elderly and strengthen their TCM awareness.2.The distance to medical treatment and the awareness rate of the family doctor contracting system will affect the willingness of elderly people to seek Chinese medicine services first;most elderly people will consider the geographical accessibility and process accessibility of medical treatment in Chinese medicine consultation, and prefer to seek medical treatment in the community. Therefore, the government should attach importance to the development of TCM services at the grassroots level, give full play to the advantages of family doctor teams in the health management of the elderly in the community, actively promote TCM health management services for the elderly, and effectively improve the ability of TCM to prevent and treat illnesses at the grassroots level.3.The attitude of the doctor, the effectiveness of the consultation and the cost of the consultation are the factors that influence the willingness of the elderly to visit Chinese medicine services in the first place. Therefore, primary health care institutions should increase the capacity and quality of their medical and nursing staff to provide better and higher quality Chinese medicine services;the government should increase the policy inclination towards Chinese medicine services and raise the level of compensation for Chinese medicine costs,so that elderly people can have access to affordable and convenient medical treatment.
