Objective: In this study,a randomized controlled study was conducted to observe the efficacy of Shengyang Yiwei Decoction combined with enteral nutrition emulsion in improving the nutritional status of patients with advanced gastric cancer with spleen deficiency and dampness stagnation,and to investigate the mechanism involved,so as to provide clinical ideas for improving the nutritional status of patients with advanced gastric cancer.Methods: Sixty patients fulfilling the inclusion criteria were divided into observation and control groups according to the random number table method.The observation group was given Shengyang Yiwei Decoction together with enteral nutrition emulsion orally,while the control group was only given enteral nutrition emulsion orally for 4 weeks as a cycle,and a total of 2 cycles were observed.The BMI、PG-SGA score,KPS score,TCM evidence score,QLQ-C30 score,serum nutritional indexes and inflammatory factors levels were recorded before and after 2 cycles of treatment in both groups,and the data were statistically analyzed using the software SPSS23.0,and the results were finally evaluated.Results:(1)BMI: BMI was effectively improved in both groups after treatment(P<0.01),with the observation group having an advantage over the control group in improving BMI between groups(P<0.01).(2)PG-SGA score: PG-SGA score improved in the control group after treatment(P<0.05)and significantly improved in the observation group(P<0.01),and the effect was better in the observation group than in the control group in com Parison between groups(P<0.01).After treatment,20 people in the observation group were removed from the malnutrition criteria(P<0.01)and 5 people in the observation group were removed from the malnutrition criteria(P < 0.05),and the comparison between groups was also statistically significant(P<0.01).(3)TCM evidence score: after treatment,the TCM symptoms in both groups improved significantly(P<0.01),but the observation group was better than the control group when compared with the two groups(P<0.01);the total effective rate of treatment in the observation group was 86.2%,while that in the control group was 57.1%,with statistically significant differences(P<0.01).(4)KPS score: After treatment,the KPS score of the control group was effectively improved compared with that before treatment(P<0.05),and the KPS score of the observation group was significantly improved(P<0.01),and the difference between the two groups was statistically significant(P<0.01).(5)QLQ-C30 score: In terms of each functional domain: the two groups improved in somatic function,cognitive function and total health status after treatment(P<0.05),in addition the observation group also showed significant improvement in emotional function(P<0.05).In relation to the individual Symptom domains: After treatment,the observation group showed a significant decrease in symptom scores for fatigue,nausea and vomiting,insomnia,loss of appetite and constipation(P<0.05),and better than that of the control group(P<0.05).(6)Nutritional indexes: in the observation group,transferrin,prealbumin,albumin and haemoglobin all showed significant improvement compared with those before treatment(P<0.05),while the change in transferrin in the control group was not significant(P>0.05),and the remaining indexes improved effectively(P<0.05);all four groups showed statistically significant comparison between groups(P<0.05).(7)Inflammatory factors level: After treatment,the levels of inflammatory factors TNF-α,IL-6,IL-1β and CRP were significantly reduced in the observation group compared with those before treatment(P<0.05),while IL-1β was not significantly improved in the control group(P >0.05),and the remaining indicators were improved(P <0.05);comparison between groups showed that the observation group had better downregulation of inflammatory factor levels than the control group(p<0.05).Conclusion: Shengyang Yiwei Decoction combined with enteral nutrition emulsion can effectively improve the TCM symptoms of advanced gastric cancer of spleen deficiency and dampness stagnation type,alleviate clinical symptoms,improve nutritional status,enhance patient’s constitution,down-regulate inflammatory indexes,reduce inflammatory response,improve the body’s anti-tumor ability and enhance the quality of life. |
