Objective:By detecting the levels of inflammatory indexs(NLRP3,IL-1β,IL-18,white blood cells,neutrophils)and uric acid,explored the expression of NLRP3,IL-1β,IL-18,white blood cells,neutrophil and uric acid in patients with essential hypertension and normal blood pressure.Analyzed the correlation between these indexs and left ventricular mass index.The aim of this study was to provide a reference for inflammatory indexs and uric acid to monitor the occurrence of essential hypertension and left ventricular hypertrophy.Methods:(1)Selected a total of 186 subjects who were hospitalized and met the inclusion criteria from November 2021 to November 2022,included 98 cases in the hypertension group and 88 cases in the control group.(2)Collected general data such as sex,age,height,weight,BMI,BSA,ethnicity,tobacco and alcohol history,blood pressure level,type of antihypertensive drugs,and family history of all subjects.Collected blood routine(white blood cells,neutrophil count,lymphocyte count),biochemical indexes(creatinine,uric acid,total cholesterol,triglyceride,high density lipoprotein,homocysteine)and cardiac indexs include Left ventricular end diastolic inner diameter(LVEDD),ventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),and left ventricular ejection fraction(LVEF),and calculated the left ventricular mass index(LVMI).(3)Determined NLRP3,IL-1βand IL-18 by Enzyme-linked immunosorbent assay(ELISA).(4)Statistical analysis was performed.Independent sample t test was used to measure the normally distributed data between the two groups.x~2test was used for statistical data.Non-normal distribution data were tested by rank sum.One-way analysis of variance was used for the data with normal distribution among groups,and Kruskal-Wallis test was used for the data with non-normal distribution,test levelα=0.05.The correlations of NLRP3,IL-1β,IL-18,white blood cells,neutrophil,uric acid and LVMI were analyzed by Pearson correlation.With LVMI as the dependent variable,the effects of NLRP3,IL-1β,IL-18,white blood cells,neutrophil and uric acid on LVMI were analyzed by stepwise linear regression.Results:(1)There were significant differences in IL-18,neutrophil count,uric acid,high density lipoprotein and homocysteine between the hypertension group and the control group(P<0.05),and they were more significant in the hypertension group;but there were no significant differences in NLRP3,IL-1βand white blood cells among groups(P>0.05).(2)There were no significant differences in IL-18,neutrophil count and uric acid among different grades of hypertension(P>0.05).(3)There were significant differences in IL-18 and white blood cells between hypertension with LVH group and hypertension without LVH group(P<0.05).(4)There were significant differences in IVST,LVPWT,LVM and LVMI between hypertension group and control group(P<0.05).(5)White blood cells was correlated with LVMI,and the difference was statistically significant(P<0.05).Conclusion:(1)IL-18,neutrophils and UA were highly expressed in essential hypertension,but it cannot be considered that NLRP3 and IL-1βwere independent of the changes and degree of blood pressure in essential hypertension patients.(2)There were differences in IL-18 and white blood cells count levels in hypertension with and without LVH,which could provide reference for early clinical detection of IL-18 and white blood cells level to indicate hypertension complicated with myocardial damage.(3)White blood cells was correlated with LVMI and was an independent factor affecting LVMI,but NLRP3,IL-1β,IL-18,neutrophil granulocyte and UA were not considered to be independent of LVMI. |
