Objectives:To understand the risk factors related to abnormal uterine artery blood flow in early pregnancy and to explore its effect on pregnancy outcome,do a good job in educating women during pregnancy and monitoring various indicators,to prevent or reduce the occurrence of adverse pregnancy outcomes and improve maternal and infant outcomes.Methods:The outpatient ultrasound system and Haitai electronic medical record system of our hospital were used to collect pregnant women who underwent uterine artery blood flow examination in the outpatient department of Bethune Second Hospital of Jilin University from September 2020 to September 2022 and aborted or delivered in our hospital.A total of 379 cases were collected,and 364 cases were selected according to the exclusion criteria.According to the diagnostic criteria,the subjects were divided into 193 cases of abnormal blood flow and 171 cases of normal blood flow.The age,pre-pregnancy BMI,education level,pregnancy history,blood type,pre-pregnancy hypertension history,and other related factors of the two groups were retrospectively analyzed,and the incidence of adverse pregnancy outcomes was compared between the two groups.Results:1.Single factor analysis of abnormal uterine artery blood flow in early pregnancy showed that age,pre-pregnancy BMI,primiparity,number of pregnancies,number of fetal arrests,history of hypertension,simple subchorionic hematoma,subchorionic hematoma combined with vaginal bleeding were associated with abnormal blood flow(P<0.05);Multivariate regression analysis showed that age>30 years old,overweight or obesity before pregnancy,history of fetal arrest,history of hypertension,simple subchorionic hematoma,subchorionic hematoma combined with vaginal bleeding were independent risk factors for abnormal uterine artery blood flow in early pregnancy.2.In the abnormal blood flow group,the incidence of hypertensive disorders complicating pregnancy(56.6%),hypothyroidism(15.9%),oligohydramnios(7.3%),abortion(5.7%),premature delivery(24.9%),FGR(9.8%),and low birth weight(34.1%)were higher than those in the normal blood flow group(P<0.05).Conclusion:1.Previous history of fetal arrest,pre-pregnancy BMI≥25.0kg/m~2,primiparity,simple subchorionic hematoma,subchorionic hematoma with vaginal bleeding,previous history of hypertension,and age>30 years old can all increase the risk of abnormal uterine artery blood flow in early pregnancy to varying degrees.2.Pregnant women with abnormal uterine artery blood flow in early pregnancy have a higher risk of hypertensive disorder complicating pregnancy,FGR,oligohydramnios,and hypothyroidism during pregnancy.3.Abnormal uterine artery blood flow in early pregnancy has a certain impact on pregnancy outcome and neonatal weight,which can significantly increase the risk of abortion,premature delivery,and low birth weight.4.For pregnant women with the above-mentioned risk factors in early pregnancy,the examination of uterine artery blood flow can be improved according to the actual situation,and the number of prenatal examinations can be increased if necessary to reduce the incidence of adverse pregnancy outcomes.
