Background: Chlamydia psittaci pneumonia is a zoonotic atypical pathogen pneumonia caused by Chlamydia psittaci,which occupies a high proportion of alveolar lavage fluid in intensive care unit patients.In the past,because it is not a routine test item and the traditional test method has low specificity,low sensitivity,slow detection speed,no clinical performance With the application of NGS,the diagnosis rate of Chlamydia psittaci pneumonia has improved,and with the timely,accurate and reasonable application of antibiotics,the morbidity and mortality rate of Chlamydia psittaci pneumonia is extremely low and the prognosis is good.Objective: By analyzing data from patients with Chlamydia psittaci pneumonia,the clinical characteristics of Chlamydia psittaci pneumonia of different severity were investigated.Methods: Retrospective analysis of clinical data of patients diagnosed with Chlamydia psittaci pneumonia in our hospital from January 2019 to January 2023,including: general data,clinical manifestations,laboratory values,imaging and other information.Results: 1.The proportion of male and winter hospitalization in severe patients were significantly higher than those in non-severe patients(P<0.05),and the length of hospitalization was significantly longer than that in non-severe patients(P<0.05).The proportion of dizziness,headache and rhabdomyolysis in severe patients was higher than that in non-severe patients,with statistically significant difference(P<0.05).WBC,CRP,PCT,neutrophils,AST,direct bilirubin,total bilirubin,BUN,creatinine,LDH,CK-MB,CK,D-dimer in severe patients were significantly higher than those in nonsevere patients(P<0.05).Lymphocyte and albumin in severe group were lower than those in non-severe group,and the difference was statistically significant(P<0.05).The proportion of lesions involving multiple lobes,bilateral lungs,left lower lobe,right upper lobe and right lower lobe in severe group was higher than that in non-severe group,and the difference was statistically significant(P<0.05).2.Chlamydia psittaci pneumonia group was significantly higher in age,length of hospital stay and winter hospitalization rate than mycoplasma pneumonia group(P<0.05).The proportion of admission in summer in psittacosis group was lower than that in mycoplasma pneumonia group,with statistical significance(P<0.05).The highest body temperature,the first episode of fever,fever,dyspnea,intolerance of cold and fatigue in psittacosis group were higher than those in mycoplasma pneumonia group,with statistical significance(P<0.05).The proportion of cough and expectoration in psittacosis group was lower than that in mycoplasma pneumonia group,with statistical significance(P<0.05).CRP,neutrophil,PCT,transaminase,direct bilirubin,total bilirubin,urea nitrogen,creatinine,creatine kinase,lactate dehydrogenase,creatine kinase isoenzyme in psittacosis group were higher than those in mycoplasma pneumonia group,and there was statistical difference(P<0.05).Lymphocyte,albumin,K+,Na+ in psittacosis group were lower than those in mycoplasma pneumonia group,with statistical significance(P<0.05).The proportion of psittacosis involving upper lobe of right lung,consolidation,pericardial effusion,air bronchus sign,pleural effusion and lymphadenopathy was higher than that of mycoplasma pneumonia group,with statistical significance(P<0.05).Conclusions: 1.In severe Chlamydia psittaci pneumonia compared with nonsevere disease,males are more common,winter onset is common,hospitalization days are long,dizziness and headache are common;inflammatory indexes are high in the severe Chlamydia psittaci pneumonia group,liver function abnormalities,renal function abnormalities,muscle damage,vascular endothelial damage,lymphopenia is more severe,and hypoproteinemia is common;severe Chlamydia psittaci pneumonia lesions often involve multiple lung lobes,bilateral lungs,and lower lobes.2.In Chlamydia psittaci pneumonia,compared with Mycoplasma pneumoniae,patients are older,have longer hospitalization days,and winter onset is common;systemic toxemia symptoms and respiratory failure are more common in Chlamydia psittaci pneumonia;cough and sputum are more common in the Mycoplasma pneumoniae group;Chlamydia psittaci pneumoniae has high inflammatory markers,abnormal liver function,abnormal renal function,muscle damage,lymphopenia is common,and hypoproteinemia,hyponatremia,and hypokalemia are often seen;CT imaging of Chlamydia psittaci pneumoniae often shows a large solid shadow,pleural effusion,pericardial effusion,and lymph node enlargement..
