| Objective:To explore the epidemiological and clinical characteristics of children hospitalized with pertussis,to improve clinicians’ understanding of pertussis in children,and to provide some basis for early diagnosis and treatment of this disease.Methods:The clinical data of 132 children with pertussis hospitalized in the First Hospital of Jilin University from January 2019 to June 2022 were collected.The date on epidemiology,clinical manifestations,ancillary examinations,treatment and prognosis of these cases were retrospectively analyzed.Results:1、The age of the children included in this study range was 4 months to15 years,with a median age of 6(4,8)years,of which 28 cases(21.2%)are≤3 years old,66cases(50.0%)are 3 to 7 years old,and 38 cases(28.8%)are>7 years old.The number of patients aged 5 and 6 increased significantly.2、Typical clinical symptoms were present: paroxysmal cough in 55 cases(41.7%),inspiratory whoops in 24 cases(18.2%);Other clinical symptoms: cough worse at night in 76 cases(57.6%),posttussive vomiting in 47 cases(35.6%).3、Comparison of clinical manifestations in different age groups:The proportion of those with cyanosis and pulmonary moist rales were higher in the ≤3 years old group than in the >7 years old group,and the difference was statistically significant(< 0.05).The proportion of those with asthmatic sounds was higher in the ≤3 years old group than in the 3~7 years old group and >7 years old group,and the difference was statistically significant( < 0.05).There was no significant difference in paroxysmal cough and inspiratory whoops among the three groups(> 0.05).4、Comparison of Bordetella pertussis infection alone and co-infection with other pathogens: Compared with simple infection group,the co-infection group had a higher proportion of fever,expectoration and lung imaging showed pneumonia,and the difference was statistically significant( < 0.05).There was no significant difference in course of disease among the two groups(> 0.05).5、Comparison of the effect of different macrolide application time: The course of disease in the group with regular use of macrolide antibiotics in the first week was shorter than that in the group with irregular use of macrolide antibiotics,and the difference was statistically significant(< 0.05).Conclusions:1 、 Pertussis is not uncommon in children older than 3 years,and preschool children are the age group with a high incidence of pertussis.2、Some children with pertussis have atypical clinical manifestations.3 、 Pertussis is prone to the combination of other pathogenic infections.The possibility of the co-infection should be noted when a child with pertussis develops a fever,but the co-infection does not prolong the course of pertussis.4、Regular application of macrolide in the first week of the course can effectively shorten the course of the disease. |
