Objective:To explore the value of neutrophil to albumin ratio(NAR)and prognostic nutritional index(PNI)in evaluating the short-term prognosis of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Materials and Methods:Retrospectively analyze the clinical data of patients diagnosed with AECOPD admitted to our hospital from January 2018 to May 2022(AECOPD group,n=100),and select the data of patients without chronic obstructive pulmonary disease who had physical examination in the hospital during the same period(health examination group,n=80),record the general data of the two groups of cases,For example, sex and age, laboratory results(lymphocyte number, neutrophil number, albumin),calculate the ratio of neutrophils to albumin(NAR),prognostic nutrition index(PNI)=serum albumin(g/L)+5 absolute lymphocyte number(*10~9/L).Patients in Group AECOPD were followed for 6 months and were classified into Group A(no acute events,n=79)based on recent acute events(hospitalization or death, readmission,or death within 6months of discharge),and Group B(no acute events,n=21).The collected data were processed through data analysis and analysis using SPSS 22.0 statistical software, comparing whether the general data,NAR and PNI of each group were statistically significant. The prognostic value of NAR,PNI and the combination of both in patients of AECOPD was compared using the receiver working curve(ROC curve).Results:1、The AECOPD group(100 cases)and the healthy physical examination group(80 cases)in general information(age,gender,etc.)P>0.05,the difference is not statistically significant; The prognosis nutrition index(PNI)in the health examination group was higher than that of the AECOPD group clearly(P<0.05);NAR in health examination group was distinct lower than AECOPD group(P<0.05).2、Patients in AECOPD group were followed up for 6 months and were divided into Group A(without acute events,n=79)and Group B(with acute events,n=21).(1)comparing the two groups, there was no difference at age(P>0.05);(2)The difference between the two groups in terms of gender was statistically significant(P<0.05);(3)Albumin,absolute number of lymphocytes,PNI in the no acute event group were obviously higher than those in the acute event group(P<0.05);(4)The NAR in the non-acute event group was distinct lower than that in the acute event group(P<0.05).3、For the ROC curve analysis of whether AECOPD patients had acute events, the area under the curve of NAR,PNI and NAR combined with PNI were0.644,0.709 and 0.753,the predictive assessment value of NAR combined with PNI is better than that for both separate applications. Conclusion:NAR and PNI have a predictive value for the occurrence of recent acute events in AECOPD patients, and the prediction accuracy and value of the two combined evaluation are higher than those of the two used alone.
