| Objective:Malnutrition is very common in COPD patients,which has a very serious impact on the clinical prognosis.The occurrence of malnutrition is closely related to inflammatory response.This study aims to explore the correlation between the blood cell related parameters and nutritional indicators in patients with COPD,so as to provide reference for clinical evaluation ofthe condition and nutritional status of patients with COPD.Method:59 patients with AECOPD admitted to the Affiliated Hospital of North Sichuan Medical College from May 1,2021 to November 30,2021,55 patients with stable COPD and 48 healthy people who were followed up in the outpatient department in the same period were selected as the control group.The name,gender,age(years),smoking history,basic diseases,height(cm),weight(kg)and other general information were recorded,and the upper arm circumference(cm)was measured triceps brachii skinfold thickness(mm).Collect laboratory test data,including neutrophils(NE),lymphocytes(LY),monocytes(MO),platelets(PLT),mean platelet volume(MPV),albumin(g/L),etc.calculate the neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and the monocyte/lymphocyte ratio(MLR)and systemic immune inflammation index(SII)were analyzed statistically.Spearman correlation analysis was used to evaluate the correlation between the blood cell parameters and nutritional indexes in patients with COPD.Results:(1)There were no significant differences in gender,age and smoking history among AECOPD,stable COPD and healthy control groups(P>0.05).(2)The levels of upper arm circumference and albumin in AECOPD group and COPD stable group were significantly lower than those in healthy control group,while the upper arm circumference and albumin in AECOPD group were lower than those in COPD stable group(P<0.05).There was no significant difference in BMI and skinfold thickness among the three groups(P>0.05).(3)NE in AECOPD group and COPD stable group was significantly higher than that in healthy control group(P<0.05);There was no significant difference in NE between AECOPD group and COPD stable group.LY in AECOPD group was lower than that in healthy control group,and the difference was statistically significant(P<0.05);MO,NLR and MLR in AECOPD group were significantly higher than those in healthy control group(P<0.05);There was no significant difference in LY,MO,NLR and MLR between COPD stable group and healthy control group,and there was no significant difference in PLT,PLR and MPV among the three groups(P>0.05).In SII level,AECOPD group was significantly higher than COPD stable group and healthy control group,and COPD stable group was higher than healthy control group,the difference was statistically significant(P<0.05).(4)NLR was negatively correlated with albumin and upper arm circumference(P<0.05);MLR was negatively correlated with albumin and upper arm circumference(P<0.05);SII was negatively correlated with albumin and upper arm circumference(P<0.05).Conclusion:The nutritional status of patients with acute exacerbation of chronic obstructive pulmonary disease is worse than that in stable stage,and there is a certain correlation between the blood cell parameters and nutritional indexes.With the aggravation of COPD patients’ condition,their inflammatory reaction is more obvious than that when the condition is stable,and their nutritional status is further deteriorated.Clinically,NLR,MLR and SII may be used to indirectly reflect the severity and nutritional status of patients. |
