| Background :Sepsis is a serious and potentially fatal disease of organ dysfunction caused by the host’s dysfunctional response to infection.Although the treatment of sepsis has developed rapidly in recent years,the morbidity and mortality of sepsis in clinical treatment remain high.The epidemiological investigation of sepsis in the intensive care unit(ICU)showed that Sepsis has an incidence of 20.6% and a 90-day fatality rate of 35.5%.The clinical manifestations of sepsis are non-specific and are easy to be confused with inflammation caused by pathogenic bacterial infection.Therefore,early diagnosis and treatment are crucial to improve the prognosis of sepsis patients.Unfortunately,there is no sensitive index in the laboratory for the diagnosis of sepsis at present,and microbial culture,as the gold standard of diagnosis,takes a long time,usually about 3-4 days.Its sensitivity is only 40%,so it is particularly important to find non-culture biomarkers with high specificity and sensitivity for early identification of sepsis.Objective:The CD64 index,presepsin and s CD163 levels of sepsis patients were detected to explore their diagnostic efficacy in sepsis,providing specific indicators for the diagnosis of sepsis.Methods:Patients diagnosed with sepsis in Jiangxi Provincial People’s Hospital from June2022 to December 2022 were selected.According to the latest guidelines for sepsis2021,patients were divided into general sepsis group and septic shock group according to the severity of sepsis,and physical examination personnel during the same period were selected as healthy control group.The CD64 index was detected by flow cytometry,and presepsin and s CD163 were detected by enzyme-linked immunosorbent assay(ELISA).The diagnostic value of each level in sepsis patients was analyzed.Results:1.The CD64 index,presepsin and s CD163 levels of sepsis patients were higher than those of healthy control group,and the CD64 index,presepsin and s CD163 levels of sepsis shock group were higher than those of common sepsis group,with statistical differences(P<0.05).2.The sensitivity,specificity and AUC of CD64 index in diagnosing sepsis were82.1%,95.2% and 0.95 respectively.The sensitivity,specificity and AUC of presepsin in the diagnosis of sepsis were 95.5%,85.7% and 0.96 respectively.The sensitivity,specificity and AUC of s CD163 for sepsis were 67.2%,90.5% and 0.75 respectively.Conclusion:1.CD64 index,presepsin and s CD163 had a certain ability to distinguish the severity of sepsis.2.CD64 and presepsin alone have a high diagnostic value for sepsis,which can provide reference for early prediction of sepsis. |
