| With the aging of Chinese population,the rational allocation of medical resources attracts more and more attention from the society.In order to provide efficient,high-quality and sustainable medical services for the masses,China launched a hierarchical diagnosis and treatment system in 2015,which divides medical services according to the severity and urgency of diseases and assigns them to hospitals at all levels for their own management,with a view to realizing complementary resources and advantages through cooperation and coordination among medical institutions at different levels,thus reducing medical costs.But in practice,the rational utilization of hospital bed resources is still a major problem.As one of the important resources of hospital,the utilization rate of hospital bed is directly related to the service ability and benefit of hospital.However,in the current medical environment,the problem of "difficult hospitalization" caused by the low utilization efficiency of bed resources still exists,which seriously affects patients’ medical experience.In order to solve this problem,this paper discusses how to optimize the allocation of bed resources under the background of hierarchical diagnosis and treatment,so as to maximize the utilization of bed resources and promote the high-quality and sustainable development of hospitals.First of all,this paper adopts the method of game theory,based on the perspective of the first "practitioner" of two-way referral doctor,starting from the doctor’s selection strategy,establishes a dynamic game model including patients and doctors,and analyzes the impact of variables such as poor prior judgment of patients,medical insurance payment ratio,and patient type on the optimal allocation of bed resources.At the same time,further explore the core variables that affect the allocation of bed resources.Secondly,the M/M/C queuing model is constructed by using the queuing theory to obtain the optimal amount of outpatient appointments,and the demand for bed resources in the coming week is predicted according to the characteristics of obstetrics and gynecology and the hospitalization rate of patients in this department in a third-class YA hospital.Finally,according to the related theory of integer programming,the optimal allocation model of hospital beds for patients with medical association is constructed,which considers the proportion of medical insurance payment and the type of patients,and aims to maximize the benefit.The results show that the type of patients and the proportion of medical insurance payment are the core variables affecting the optimal allocation of hospital bed resources.The optimal bed allocation model established in this paper can ensure the health benefits of patients and reflect the public welfare of public hospitals by arranging the time and order of patients’ admission,and increase the hospital’s income by at least 8599 yuan. |
