Objective:To explore the sleep problems of children in the health examination from 0-23 months in the outpatient department of child care and analyze the influencing factors,so as to provide a certain clinical basis for future research.Research Objects and Methods:In this study,children aged 0-23 months who underwent physical examination in the child care clinic of the Affiliated Hospital of Yan’an University from December 2021 to December 2022 were selected as the subjects of investigation.Sleep was assessed with a child’s sleep-related questionnaire,which prepared according to the "Guidelines for Sleep Health of Children Aged 0-5" issued by the National Health and Family Planning Commission of the People’s Republic of China in 2017,the diagnostic criteria used in the2006 National Maternity and Child Health Center’s research on "the Status of Sleep Habits and Sleep Disorders of Children Aged 0-5" and the " A Brief Screening Questionnaire for Infant Sleep Problems(BISQ)" prepared by Sadeh et al.Children with sleep problems who met the inclusion and exclusion criteria were included as a sleep problem group,while children without sleep problems were included as a control group.According to the Children’s Sleep Hygiene Guidelines of 2017,children are divided into0-3 months,4-11 months,and 12-23 months.To analyze children’s sleep problems and influencing factors.Outcome:1.General data:A total of 275 chrildren were included in this study.There were 92 children in the sleep problem group,including 50 males(54.3%),42 females(45.7%)and183 children in the control group,which includes 95 males(51.9%),88 females(48.1%).In the sleep problem group,36(39.1%)cases aged 0-3 months,40(43.5%)cases aged 4-11 months,16(17.4%)cases aged 12-23 months;in control group,66(36.1%)cases aged 0-3 months,68(37.2%)cases aged 4-11 months,49(26.8%)cases aged 12-23 months.2.Sleep problems2.1 There were significant differences in sleep time at night,daytime and whole day among different age groups(p<0.05);And,there was significant difference in the number of night wakes between different age groups(p<0.05),Children aged 0-3 months wake up more frequently at night than children aged 4-11 months and more than children aged12-23 months;Evidently,there was a statistically significant difference between the sleep problem group and the non-sleep problem group in the time of falling asleep and the times of waking up at night(p<0.05).2.2 In this study,24(8.7%)children were sleep-deprived,and 2(0.7%)children sleep time more than the recommended time;4(1.5%)children fell asleep before19:00,154(56.0%)children fell asleep after 21:00;14(5.1%)children were awake for more than one hour at night.2.3 In this study,difficulty in falling asleep was the most common,followed by open mouth breathing,frequent wake-up at night,spastic twitching/shaking of limbs,snoring,more daytime sleep than night sleep,early sleep,and apnea.3.Univariate analysis of sleep problems: According to the results of univariate analysis,the child’s gestational age,feeding mode,sleeping mode,whether the light was turned on in the room at night,whether the sleeping environment was quiet,whether the caregivers often change,the use of vitamin AD/vitamin D,the mother’s mood during pregnancy,the mother’s sleep problems(tooth grinding,snoring,sleeping,insomnia),the father’s sleep problems(tooth grinding,snoring,sleeping,insomnia),the parents’ educational level,the average monthly income of the family and family structure was associated with sleep problems occurrence,and the difference was statistically significant(P<0.05).4.Multivariate logistic regression analysis of sleep problems:Multivariate logistic regression analysis showed that artificial feeding,the caregiver is fixed,the light is sometimes turned on in the room at night,the light is often turned on in the room at night,the sleep environment is noisy,the sleep environment is little quiet,and the mother has sleep problems were statistically significant(P<0.05).Conclusion:1.The most common sleep problems of children aged 0-23 months are difficulty in falling asleep and mouth breathing.2.The caregiver is fixed and artificial feeding is used as protective factors for children sleep,while the light is sometimes turned on in the room at night,the light is often turned on in the room at night,the sleep environment is noisy,the sleep environment is little quiet,and the mother’s sleep problems are its risk factors.
