ObjectiveTo study the correlation between serum S100 B protein,lipoprotein phospholipase A2(Lp-PLA2)levels and cognitive impairment in cerebral small vessel disease(CSVD),and to preliminarily explore the relationship between serum S100 B protein,lipoprotein phospholipase A2(Lp-PLA2)levels and the total MRI burden score of CSVD.It provides a new research direction for the prevention and treatment of CSVD.methodA total of 120 patients with cerebral small vessel disease who were hospitalized in the Department of Neurology of our hospital from December 2021 to January 2023 selected as the case group,and all patients underwent brain magnetic resonance imaging.The patients were examined by magnetic resonance imaging(MRI),and the Montreal cognitive assessment(MOCA)scale was used.According to the Montreal Cognitive assessment(MOCA)score,the case group was divided into Non-CI group and CI group.According to the total MRI burden score,the case group was divided into mild group and moderate to severe group.100 cases of physical examination in our hospital during the same period were randomly selected as the control group.The gender,age,hypertension,diabetes,smoking history,drinking history,uric acid,Hcy and other indicators of the case group and the control group were collected,and the serum S100 B protein and lipoprotein phospholipase A2(Lp-PLA2)levels were detected.The differences of serum S100 B protein,lipoprotein phospholipase A2(Lp-PLA2),uric acid and Hcy levels between the case group and the control group were analyzed by t test.The differences of serum S100 B protein,lipoprotein phospholipase A2(Lp-PLA2),uric acid and Hcy in the Non-CI group,the CI group,the mild group and the moderate to severe group in the case group were compared.Binary Logistic regression analysis was used to compare the relationship between serum S100 B protein,lipoprotein phospholipase A2(Lp-PLA2)and cognitive impairment of cerebral small vessel disease.Spearman correlation analysis was used to analyze the correlation between serum S100 B protein,lipoprotein phospholipase A2(Lp-PLA2),MOCA score and total MRI burden score.Receiver operating characteristic(ROC)curve was drawn to evaluate the efficacy of serum S100 B protein and lipoprotein phospholipase A2(Lp-PLA2)levels in evaluating the risk of CSVD cognitive impairment.Results1.Compared with the control group,the serum S100 B protein,Lp-PLA2,uric acid and Hcy levels in the case group were significantly higher than those in the control group,with a statistically significant difference(P <0.05).2.Binary Logistic regression analysis based on CSVD cognitive impairment as the dependent variable and serum S100 B protein and Lp-PLA2 level as independent variables showed that serum S100 B protein and Lp-PLA2 were closely related with the occurrence of CSVD cognitive impairment(OR = 1.882,1.003,P <0.01),and serum S100 B protein and Lp-PLA2 were risk factors for CSVD cognitive impairment.3.Serum S100 B protein,Lp-PLA2,uric acid and Hcy levels were significantly different between cognitive impairment(CI),and serum S100 B protein,Lp-PLA2,uric acid and Hcy levels were higher in the CI group were higher than those in the Non-CI group(P <0.05).4.The serum levels of S100 B protein and Lp-PLA2 were significantly different between the mild and moderate to severe MRI load groups,with the serum levels of S100 B protein and Lp-PLA2 significantly higher in the moderate to severe group than the mild group,that is,the higher the total MRI load score,the higher the serum level of S100 B protein and Lp-PLA2(P <0.01).5.Spearman correlation analysis showed a positive relationship between S100 B protein and Lp-PLA2 level and MRI total load score(P <0.01)and a negative correlation with MOCA score(P <0.05),that is,the more severe the cognitive impairment,the higher the MRI total load score,the higher the serum S100 B protein and Lp-PLA2 level.6.ROC curve analysis showed that the AUC value of serum S100 B protein,Lp-PLA2 level for the risk of CSVD was 0.766 for cognitive impairment,0.650,respectively,and the AUC value of the combined test was 0.809,which showed higher sensitivity than that of serum Lp-PLA2 and higher accuracy than serum S100 B protein.Conclusion1.Serum S100 B protein and Lp-PLA2 levels are significantly associated with the occurrence of CSVD,and increased serum S100 B protein and Lp-PLA2 levels may be a risk factor for cognitive impairment in CSVD.2.Serum S100 B protein and Lp-PLA2 levels are closely related to cognitive impairment and MRI total load score in CSVD patients,showing a positive correlation,that is,the more severe the cognitive impairment,the higher the MRI total load score,and the higher the serum level of S100 B protein and Lp-PLA2.3.Serum S100 B protein and Lp-PLA2 levels may be predictors of cognitive impairment in CSVD,and have good predictive value for assessing the occurrence of cognitive impairment in CSVD. |
