Objective:In this study,we used taijiquan and fitness qigong six-word formula as exercise modalities to increase patient compliance,improve exercise health beliefs and self-management ability of type 2 diabetic patients through easy-to-understand diet education and patient management,and observe the intervention of the two exercise modalities on glucolipid metabolism,partial fitness and quality of life of type 2 diabetic patients.The aim of this study is to find an exercise program that can be adapted to the individual,time and place.Methods : Fifty-four patients with type 2 diabetes were selected according to the inclusion and exclusion criteria,and randomly divided into three groups: the control group,the taijiquan group,and the fitness qigong six-word formula group.The taijiquan and fitness qigong six-word formula groups were intervened 5 times a week for 1 hour each time for12 weeks,and the exercise process was improved and dietary reference was provided.The patients’ blood glucose,blood lipids,some physical fitness indicators and quality of life indexes were evaluated before and after 12 weeks of the intervention.Paired-sample t-test was used to compare the effects before and after the intervention within the study group,and one-way analysis of variance was used to compare between the three groups.Results:(1)Blood glucose indexes: within group comparison,after 12 weeks of intervention,patients in the Taijiquan group showed significant reductions in fasting blood glucose(FBG)and glycated haemoglobin(Hb A1c)(P < 0.05),while patients in the fitness qigong six-word formula group showed significant reductions in fasting blood glucose(FBG)after intervention(P<0.05),while there was no significant difference in glycated haemoglobin(Hb A1c)after 12 weeks of intervention(P>0.05).There was no significant difference in glycosylated haemoglobin(Hb A1c)(P>0.05),and almost no significant change in blood glucose indicators in the control group after 12 weeks(P>0.05);comparison between groups: there was a significant difference in all indicators between the control group and Taijiquan(P<0.05),and compared with fitness qigong six-word formula,there was a significant difference in fasting blood glucose(P<0.05).There was a significant difference in fasting blood glucose(P<0.05)and no significant difference in Hb A1c(P>0.05);the fitness qigong six-word formula group and the Taijiquan exercise group both showed improvement in FBG indexes,with no significant difference(P>0.05),but the difference in Hb A1 c was statistically significant(P<0.05).(2)Blood lipid indexes: for intra-group comparison,after 12 weeks of intervention,there were almost no significant differences in triglycerides(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)in the control group compared with the pre-intervention period(P > 0.05);there were significant differences in lowering TG,TC and LDL-C in the Taijiquan group(P <0.05),and the fitness qigong six-word formula group mainly showed a significant increase in HDL-C(P < 0.05)and decrease on LDL(P < 0.01)after the intervention,but no significant difference in TG,and no significant difference in TC although there was an increase(P > 0.05).Comparison between groups: Compared with the control group after 12 weeks of intervention,the fitness qigong six-word formula group showed no significant differences in TG and TC(P > 0.05),and significant differences in HDL-C and LDL-C(P <0.05),while the Taijiquan group showed significant differences in TG,TC and LDL-C(P <0.05),and The Taijiquan group showed significant differences in TG,TC and HDL-C,but not in LDL-C(P>0.05).(3)Some physical fitness indicators: Within-group comparison: the patients in the fitness qigong six-word formula group showed no significant increase in sitting forward bending,standing on one leg with eyes closed and lower limb muscle strength tests compared with those before the intervention(P>0.05),but there was a great change in lung capacity(P<0.05);the taijiquan group showed a significant increase in sitting forward bending,standing on one leg with eyes closed and lower limb muscle strength tests compared with those before the intervention(P<0.05),and there was no significant difference in lung capacity although there was some improvement(P>0.05);the control group did not have significant changes in physical fitness indicators after the intervention compared with those before the intervention(P>0.05).Comparison between groups:compared with the taijiquan group,there was a significant difference between the fitness qigong six-word formula group and the control group in sitting forward bending,standing on one leg with eyes closed,and lower limb muscle strength test(P<0.05),and for the change in lung capacity,there was a significant difference between the control group,the taijiquan group and the fitness qigong six-word formula group(P<0.05).There was no significant difference between the fitness qigong six-word formula group and the control group(P>0.05),except for lung capacity(P < 0.05).(4)Quality of life scale(SF-36): intra-group comparison: there was no significant change in the SF-36 indicators in the control group before and after the intervention(P>0.05);there was no significant difference in the taijiquan group before and after the intervention in physical function PF(P=0.055),somatic pain BP(P=0.116)and social function SF(P=0.136),while there was significant improvement in RP,GH,VT There was a significant increase in RP,GH,VT,RE,and MH(P < 0.05);the fitness qigong six-word formula group showed some improvement in somatic pain BP(P = 0.084),general health status GH(P = 0.268),and mental health MH(P = 0.137),but the difference was not statistically significant,while RP,SF,RE were significantly higher(P < 0.05)compared to pre-intervention and there was a highly significant improvement in VT scores(P < 0.01).The control group did not improve significantly in all dimensions of quality of life(P>0.05);comparison between groups: in terms of physiological functions RP and RE,there was a significant difference between the control group and the Taijiquan and fitness qigong six-word formula groups(P<0.05).In terms of GH and MH,there was a significant difference between the Taijiquan group and the control group and the fitness qigong six-word formula group after the intervention(P<0.05).Conclusion:(1)In terms of blood glucose and blood lipids,taijiquan exercise and the fitness qigong six-word formula had an improvement on the blood glucose and blood lipids of type2 diabetic patients.However,the effect of taijiquan exercise on blood glucose and lipid indexes was greater than that of the fitness qigong six-word formula group.(2)In some physical fitness indicators,Taijiquan exercises were more advantageous in improving patients’ lower limb balance,trunk flexibility and lower limb strength,while fitness qigong six-word formula was more advantageous in improving patients’ lung function.This suggests that patients can choose a more appropriate form of exercise according to their needs.(3)In terms of quality of life,both taijiquan and fitness qigong six-word formula can improve the overall quality of life of patients with type 2 diabetes. |
