| Objective: To explore the serum levels of Syndecan-1 and N-terminal pro-brain natriuretic peptide(NT-pro BNP)in patients with chronic heart failure(CHF),and to provide a reference for improving the diagnostic efficiency of chronic heart failure.Methods:According to the 2021 European Society of Cardiology guidelines for the diagnosis and treatment of acute and chronic heart failure,115 patients with chronic heart failure in the cardiovascular medicine department of Shaoyang Central Hospital were selected as the case group and divided into the group with reduced ejection fraction(HF with reduced ejection fraction,HFr EF),the group with mildly reduced ejection fraction(HF with mildly reduced ejection fraction,HFmr EF),and the group with preserved ejection(HF with preserved ejection fraction,HFp EF).Thirty-five volunteers without heart failure of comparable age in the same period were also collected as the control group.The general clinical data,serum biochemical indexes(cardiac enzymes,glutamate transaminase,lipids,creatinine,etc),echocardiographic data,and serum SDC1 and NT-pro BNP levels of the above study subjects were collected.The statistical software SPSS 25.0 was used to analyze the differences in general data,serum biochemical indexes,echocardiographic,SDC1 and NT-pro BNP serum levels between the case group and the control group.Results:1.Glutathione transaminase,creatinine,uric acid,myoglobin,creatine kinase isoenzyme,and the diameter of heart chamber were higher in the case group than in the control group,and LVEF and total cholesterol were lower than in the control group,with statistically significant differences(P<0.05).2.The levels of serum NT-pro BNP and SDC1 were higher in all three types of CHF patients than in the control group,and the differences were statistically significant(P<0.05),and there were differences in serum NT-pro BNP levels in various types of CHF patients(P<0.05),with the highest in the HFr EF group,followed by the HFmr EF group,and the lowest in the HFp EF group;serum SDC1 levels were lower in the HFp EF group than in the HFmr EF and HFr EF groups(P<0.05),but serum SDC1 levels were not different between the HFmr EF and HFr EF groups(P>0.05).3.Pearson correlation analysis showed that ln(NT-pro BNP)and ln(SDC1)were negatively correlated with ln(LVEF)(r=-0.496,-0.297,P<0.05)and ln(NT-pro BNP)was positively correlated with ln(SDC1)(r=0.298,P<0.05).4.ROC curve analysis showed that the area under the curve for the diagnosis of CHF using NT-pro BNP alone was 0.855(95%CI: 0.792-0.918,P < 0.05),and the area under the curve for the diagnosis of CHF using SDC1 alone was 0.808(95% CI:0.726-0.890,P < 0.05),while the combined diagnosis of CHF showed an area under the curve of 0.896(95% CI: 0.839-0.953,P<0.05).Conclusion:SDC1 and NT-pro BNP both have diagnostic value for chronic heart failure,but the diagnostic efficacy of SDC1 is inferior to that of NT-pro BNP,and the application of both can improve the diagnostic efficiency for chronic heart failure. |
