Objective:To detect the serum and synovial fluid levels of Matrixmetalloproteinase3(MMP-3)and Matrixmetalloproteinase13(MMP-13)expression,in patients with knee gouty arthritis(Knee Goutyarthritis,KGA),knee osteoarthritis(Knee Osteoarthritis,KOA),knee gout combined with osteoarthritis(KGA combined with OA group).understand the difference of MMP-3 and MMP-13 expression in KGA and KOA,analyze MMP-3 and MMP-13 in joint inflammation and Significance in cartilage metabolism,explore the relationship between KGA and KOA.Methods: KGA and KOA patients were selected as the research subjects.KGA patients with knee X-ray K&L grade ≥ 1 were defined as KGA combined OA group,and normal knee joints were used as control.According to the purpose of this study,the inclusion and exclusion criteria were set,and a total of 237 subjects were collected according to the inclusion and exclusion criteria.There were 45 cases in the normal control group,45 cases of serum and no synovial fluid samples;57 cases of KGA group,32 cases of serum collection,49 cases of synovial fluid,24 cases of both serum and synovial fluid;69 cases of KOA group,33 cases of serum collection,63 cases of synovial fluid,27 cases had both serum and synovial fluid;KGA combined with OA group of 66 cases,35 cases of serum,52 cases of synovial fluid,21 cases of both serum and synovial fluid.Enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of MMP-3 and MMP-13 in serum and synovial fluid of each group,and SPSS20.0 software was used for statistical analysis.Results: 1.MMP-3 was significantly expressed in the synovial fluid of KGA group,KOA group and KGA combined OA group.The level of MMP-3 in the KGA group was higher than that in the KOA group,and the difference was statistically significant;the MMP-3 level in the KGA combined OA group was higher than that in the KGA and KOA groups,and the difference was statistically significant.To sum up: the level of MMP-3 in synovial fluid from high to low was: KGA combined with OA group>KGA group>KOA group.2.The expression of MMP-13 was different from that of MMP-3.The expression of MMP-13 was only detected in the synovial fluid of KOA group and KGA combined OA group.The level of MMP-13 in the KGA combined OA group was higher than that in the KOA group,and the difference was statistically significant.To sum up: the level of MMP-13 in synovial fluid from high to low was: KGA combined with OA group>KOA group>KGA group(0)3.In the KGA group,the level of MMP-3 in the joint fluid of those with frequent gout attacks(≥2 times/year)was higher than that of those with a single attack(1 time/year),and the difference was statistically significant.In the KGA combined OA group,76.9%(40/52)of the patients had frequent gout attacks(≥2times/year),which was higher than 52.5%(27/49)of the KGA group,and the difference was statistically significant.There was no significant difference in the levels of MMP-3 and MMP-13 in the synovial fluid of KOA groups with different K&L grades.4.The serum MMP-3 level in each lesion group was significantly higher than that in the control group,the KGA combined OA group was higher than the KGA group and the KOA group;the KGA group was higher than the KOA group.Serum MMP-3levels from high to low were: KGA combined with OA group>KGA group>KOA group>control group.The expression of MMP-13 was only detected in serum of KOA group and KGA combined OA group,and the level of MMP-13 in KGA combined OA group was higher than that in KOA group.Serum MMP-13 levels from high to low were KGA combined OA group>KOA group>KGA group(0)and control group(0);the serum levels of MMP-3 and MMP-13 in the same group were significantly lower than in the synovial fluid,the difference were statistically significant.Conclusion:1.MMP-3 is highly expressed in KGA group,KOA group and KGA combined OA group,MMP-3 may be more closely related to joint inflammation.2.MMP-13 was only highly expressed in KOA group and KGA combined OA group,MMP-13 may be more closely related to cartilage degradation.3.The presence of GA may accelerate the progression of OA,and the presence of OA may make the joints more prone to GA attacks. |
