In recent years,more than 40 kinds of infectious diseases have been reported in the camps,which have had an essential impact on the daily-life and training of the troops.The incidence of key infectious diseases such as tuberculosis,chickenpox,and viral hepatitis remains high.Respiratory infectious diseases are increasing significantly in the outbreak of infectious diseases in the troops.Once an outbreak formed,it would affect the combat effectiveness of our army.The health literacy is an ability for individual to obtain,process and understand basic health information,and the level of health literacy affects the individual’s ability to maintain and promote his own health in a certain extent.Therefore,studying the health literacy of officers in armed forces is of great significance to improve the personal health of military members and the combat effectiveness of the army.Previous report have been studied the effectiveness of the assessment of infectious disease health literacy of officers and soldiers in grass-roots units by using the questionnaire of residents’ infectious disease health literacy questionnaire.The results revealed that the scale showed low credibility and validity.The probable reasons might include special geographical environment,diverse in the performance of tasks,and highly concentrated residence,which made the spectrum of infectious diseases in the troops different from those of ordinary residents.This led to the differences in the corresponding health literacy of infectious diseases for officers in armed forces.Therefore,the content of the infectious disease health literacy assessment for ordinary residents is not fully applicable for the officers in armed forces.There is no infectious disease health literacy evaluation questionnaire for officers in armed forces.Therefore,it is necessary to develop evaluation tools of the infectious disease health literacy for officers and soldiers in grass-roots units,and then investigate the level of infectious disease health literacy among officers and soldiers,so as to provide strong theoretical support and practical guidance for the prevention education of infectious diseases.Objective1 To clarify the connotation of infectious disease health literacy of officers and soldiers in grass-roots troops,to construct a conceptual model of infectious disease health literacy in grass-roots troops.2 To develop a questionnaire on infectious disease health literacy of officers and soldiers in grass-roots troops,which provides an effective tool for assessing the infectious disease health literacy.3 To investigate the current situation and influencing factors of infectious disease health literacy of officers and soldiers in grass-roots troops,and to provide reference for the formulation of intervention measures in the future.Methods1 The entry pool of infectious disease health literacy questionnaire for officers and soldiers in grass-roots units was formed through literature research,expert group meeting and qualitative interview.2 Relevant analysis was carried out to establish the initial questionnaire of infectious disease health literacy of grass-roots army officers and soldiers using Delphi expert correspondence method.A questionnaire survey was conducted among 444 officers and soldiers of grass-roots units.The final draft of the questionnaire on infectious disease health literacy among officers and soldiers in grass-roots units was formed through project analysis and exploratory factor analysis,A total of 303 officers and soldiers in grass-roots units were selected,and the questionnaire on infectious disease health literacy in grass-roots officers and soldiers was issued,and the internal consistency and split half reliability of the questionnaire were tested.3 Other 649 soldiers randomly selected from grass-roots units were investigated by using the questionnaire on infectious disease health literacy among officers and soldiers in grass-roots units.Descriptive analysis,analysis of variance,multiple stepwise regression and other statistical methods were used to clarify the current situation of infectious disease health literacy of the research objects,and the impact factors of infectious disease health literacy were found.Results1.A questionnaire on the infectious disease health literacy among officers and soldiers in grass-roots units was formed.1.1 Preliminary construction results of infectious disease health literacy questionnaire for officers and soldiers in grass-roots unitsA questionnaire with 5 dimensions and 31 items was finally formed through literature research,qualitative interviews and correspondence with Delphi experts,as well as item analysis and exploratory factor analysis on the questionnaire data of 444 members in grass-roots troops,.The five dimensions of the questionnaire are:knowledge of infectious disease prevention and control of officers and soldiers at the grassroots level;infectious disease prevention and control belief of officers and soldiers at the grass-roots level;infectious disease prevention and control behavior of officers and soldiers at the grass-roots level;camp management of common infectious diseases in the army and personal response to common infectious diseases in the army.1.2 Validation results of infectious disease health literacy questionnaire for officers and soldiers in grass-roots unitsThe reliability and validity of the "Infectious Diseases Health Literacy Questionnaire for Officers and Soldiers in Grass-roots Forces" was tested.The overall Cronbach’s alpha coefficient of the questionnaire was 0.896,the Cronbach’s alpha coefficient of the five dimensions was between 0.743-0.902,the overall split-half reliability was 0.761,and the split-half reliability of the five dimensions was between 0.732-0.917 and the test-retest reliability was 0.824(P<0.001).The content validity was 0.969,and the confirmatory factor analysis showed that the cumulative variance contribution rate of the extracted 5 common factors was 59.210%.2 Analysis on the current situation of infectious disease health literacy of officers and soldiers in grass-roots unitsThe total score of the infectious disease health literacy of the officers and soldiers in the grass-roots troops was 131.84±12.66,and the total score was 85.06%,indicating that the infectious disease health literacy of the officers and soldiers in the grass-roots troops was at an upper-middle level.3 Influencing factors of infectious disease health literacy of officers and soldiers in grass-roots units3.1 Single factor analysis resultsThere was significant difference in the scores of infectious disease health literacy among different length of military service(P<0.05).The results of inter group comparison showed that the scores of infectious disease health literacy in 11-15 year group and 16-20 year group were lower than those in 6-10 year group,and the difference was statistically significant(P<0.05).There was significant difference in the scores of infectious disease health literacy among people with different family income(P<0.05).The post pairwise comparison results showed that the infectious disease health literacy score of people with ≤2000 RMB per capita family income per month was lower than those of 4801-6000 RMB group,2001-4800 RMB group and>6001 RMB group.There was a statistically significant difference in the infectious disease health literacy scores among people with different cognition of their own physical status(P<0.05).Post-hoc comparison results showed that the infectious disease health literacy scores of the self-assessed poor physical condition group were lower than those of other groups.The scores of infectious disease health literacy in the self-assessed modest physical condition group were lower than those in the self-assessed good physical condition group.The score of infectious disease health literacy for people with family medical background was higher than that without family medical background.The score of infectious disease health literacy of those who had studied the knowledge of infectious diseases was higher than that of those who had not studied the knowledge of infectious diseases.The differences were statistically significant(P<0.05).3.2 Results of multiple stepwise regression analysisTaking the total score of health literacy as the dependent variable and the general demographic data with statistical significance in univariate analysis as the independent variable,multiple stepwise linear regression analysis was carried out.The results showed that length of military service,self-assessment of health status and knowledge of infectious diseases were the main influencing factors of infectious disease health literacy of grass-roots military soldiers.Conclusions1 The questionnaire of infectious disease health literacy of grass-roots army soldiers developed in this study has good reliability and validity,and can be used as an evaluation tool of infectious disease health literacy of grass-roots army soldiers.2 The infectious disease health literacy of soldiers in grass-roots units is generally above the medium level.Military service length,self-assessment of health status and the knowledge of infectious diseases are the main influencing factors of infectious disease health literacy of grass-roots military soldiers.
