| Objectives To study the relationship between lipoprotein a [Lp(a)],fibrinogen(FIB),Ddimer(D-D)and total cerebral small vessel disease(CSVD)burden and cognitive function in patients with CSVD,and to provide new ideas for the prevention and treatment of CSVD and cognitive impairment.Methods The complete clinical data of 201 patients with CSVD admitted to Hebei General Hospital from October 2018 to December 2021 were collected,and the patients were divided into 150 cases in the mild to moderate burden group(0-2 points)and 51 cases in the severe burden group(3-4 points)according to results of the total CSVD burden,and then divided into 140 cases in the cognitive normal group and 61 cases in the cognitive impairment group according to mini-mental state examination.Statistical methods such as t test,logistic regression and Spearman correlation analysis were applied to investigate the relationship between Lp(a),FIB,D-D and total CSVD burden and cognitive function,and to find other factors affecting total CSVD burden and cognitive impairment.Results 1 Age,homocysteine(Hcy),FIB,and D-D levels were higher in the severe burden group than in the light to moderate burden group,and the proportion of males and patients with hypertension was higher than in the light to moderate burden group,with statistically significant differences(P<0.05),and the difference in Lp(a)levels between the two groups was not statistically significant(P>0.05).Logistic regression analysis showed that age,Hcy,and FIB on total CSVD burden were statistically different(P<0.05);Spearman’s correlation analysis showed that FIB was positively correlated with total CSVD burden(r=0.417,P<0.001).2 Age,admission SBP,FIB,D-D,and total CSVD burden levels were higher in the cognitively impaired group than in the cognitively normal group,and the differences were statistically significant(P<0.05),and Lp(a)was not statistically significant between the two groups(P>0.05).Logistic regression analysis showed that the effects of total burden of FIB and CSVD on cognitive impairment were statistically different(P<0.05).Spearman’s correlation analysis showed that FIB was associated with montreal cognitive assessment(Mo CA)total score and its subscales(visuospatial and executive function,attention,naming,language,abstraction,delayed recall and orientation)were negatively correlated(P<0.05).Conclusions 1 In patients with CSVD,age,FIB and Hcy may be independent risk factors for total CSVD burden,and FIB,total CSVD burden may be independent risk factors for cognitive impairment.2 The higher the level of FIB and the heavier the total CSVD burden in patients with CSVD,the more pronounced the cognitive impairment.Figure 0,Table 5,Reference 227... |
