Study 0n Correlation Between Cognitive Function And Electroencephalogram In Patients With Cerebral Small Vessel Disease

PurposeTo analysis the relationship between the cognitive dysfunction and the characteristics of the electroencephalogram(EEG)in patients with CSVD,and to prviode objective basis for discovering cerebral small vascular cognitive impairment(CSVCI)in early phase and mastering the disease progress.MethodsRetrospectively analysis the 81 patients diagnosed CSVCI who came from the memory clinic and inpatient care in Bethune First Hospital of Jilin University from January 2015 to January 2017.Their clinical date,neuropsychological test scores and4 h video-EEG results should be recorded.Acrording the diagnosis standard from Ds M~IV,patients were divided into VCIND and Va D.Use SPSS19.0 software for statistical analysis.Result(1)There was no difference in the age,gender,education level and the proportion of hypertension,diabete,hyperlipemia,smoking and alcohol consumption among the VCIND group and the Va D group(P > 0.05).(2)The total scores and component scores of MMSE,Mo CA,MES between the VCIND group and the Va D group were significantly different(P<0.05).(3)In the study normal EEG rate was 11.1%,mildly abnormal EEG was 32.1%,moderately abnormal EEG was 48.1%,severely abnormal EEG was 8.6%.In the VCIND group 17.6% EEG was nomal,mildly abnormal EEG was 35.3%,moderately abnormal EEG was 47.1%,severely abnormal EEG was 0.0%.And Va D was 6.4%,29.8%,48.9%,14.9% respectively and they had statistical significance(P <0.05).(4)Abnormal EEG that contain background EEG activities was 11.1%,The EEG possess epileptiform discharges(anomalous activity)was 18.5%,there was 49.4%EEG both had the two types of exceptions in the CSVCI.The abnornal EEG rate among VCIND group was 35.3%,23.5%,23.5% respectively;And the Va D group was10.6%,14.9%,68.1% respctively.There was a distinct difference between the two group.(5)In our 81 cases 32.1% had no anomalous activity in their EEG and 27.2% had a small amount of anomalous activity,30.9% had the medium amount anomalous activity and a large number of anomalous activity in EEG was 9.9%.In the VCIND group 52.9% had no anomalous activity,32.4% had a small amount of anomalous activity,14.7% had the medium amount anomalous activity;In the Va D group was17.0%,23.4%,42.7% and 17.0% respectively.The two groups had statistically significant difference(P < 0.01).(6)Compared the EEG general characteristics among the VCIND group and Va D group,The background EEG frequency was slower and the slow-wave activity among background and(or)focal and abnormal rhythms generalization and adjusting amplitude modulation and epileptiform discharges(anomalous activity)were more than the VCIND group,and there has statistical significance(P < 0.05).(7)The patients who has normal,mild abnormal,moderate and severe abnormal EEG obtain different MMSE(23.89±4.137 vs 22.15±5.936 vs 21.64±5.480 vs14.43±4.577),Mo CA(18.78±5.333 vs 16.58±5.907 vs 15.36±5.603 vs 8.57±1.272),MES(63.14±17.790 vs 56.50±23.376 vs 54.57±22.909 vs 33.00±14.434),and ADL(26.22±6.037 vs 30.46±13.252 vs 31.36±12.592 vs 47.43±15.757)(P < 0.05).The patients who has without,or small,medium and large amount anomalous activity EEG also obtain different MMSE(24.65±4.039 vs 22.05±5.481 vs 19.88±5.630 vs14.13±4.324),MOCA(18.69±4.488 vs 15.82±5.395 vs 14.24±6.112 vs 8.63±3.159),MES(66.23±16.535 vs 57.42±20.868 vs 46.33±24.259 vs 34.38±17.808),ADL(26.00±5.987 vs29.95±13.415 vs 35.08±13.687 vs 46.38±17.533)(P < 0.05).Conclusion(1)With the development of the disease,the executive function,memory function and the language ability and so on various cognitive domain get worse and worse in the patients who suffer from CSVCI.(2)The abnormal EEG rate is high in the patients with CSVCI,and the light and moderate abnormal is major component.With the degree of cognitive dysfunction,increased the proportion of abnormal EEG and abnormality degree aggravating.The EEG in the diagnosis of CSVCI has prominent sensitivity,and it can reflect the degree of cognitive dysfunction.(3)Abnormal background EEG activities and epileptiform discharges(anomalous activity)are two types of exceptions among patients who with the CSVCI.And they exist in isolation or in combination.Abnormal background EEG activities play a leading role in VCIND group.To deeper cognitive impairment,abnormal background EEG activities and anomalous activity(epileptiform discharges)exist in combination.(4)With the increase in the degree of cognitive dysfunction,frequency EEG background is slower,and background EEG or focal slow-wave activity,abnormal rhythms generalization and adjusting amplitude modulation and the proportion of focal abnormal wave is also increasing.(5)EEG in patients with CSVCI may have small,medium or large amount of anomalous activity,and small or medium anomalous activity be in the majority.As the degree of cognitive impairment is aggravating,medium and large anomalous activity VEEG ratio increasing.(6)EEG classification and the number of anomalous activity(epileptiform discharges)were correlated with cognitive scale and daily life ability.
