Objectives To retrospectively analyze the clinical data of patients with thyroid nodules in our hospital,and to explore the neutrophil-to-lymphocyte ratio(NLR),platelet-tolymphocyte ratio(PLR),platelet distribution width(PDW),red blood cell distribution width(RDW)and the relationship between benign and malignant thyroid nodules.To explore the diagnostic efficacy of a single index and a combination of multiple indexes in the diagnosis of benign and malignant thyroid nodules,in order to provide a simpler,faster and easier method for the preoperative diagnosis of thyroid nodules as a supplement and reference.Methods The clinicopathological data of patients with thyroid nodules who were treated in the Department of Oncology,North China University of Science and Technology Affiliated Hospital from January 1,2016 to December 31,2019 were collected.Calculate NLR,PLR values.The subjects were divided into benign nodule group and malignant nodule group.To analyze the relationship between benign and malignant thyroid nodules and NLR,PLR,PDW,RDW,BMI,gender,age,smoking,drinking,hypertension,diabetes,hyperlipidemia,nodule diameter distribution,multifocality,combined thyroiditis rate,nodule unilateral rate,combined goiter rate,number distribution of nodules and other indicators.Logistic regression was used to analyze whether each factor was a risk factor for thyroid nodules to be malignant nodules.The receiver operating characteristic curve was used to analyze the diagnostic efficacy of preoperative NLR,PLR,PDW,RDW and the combination of each index.Results 1 PDW level,age distribution,BMI level,diabetes mellitus,hyperlipidemia,nodule size distribution,multifocality,thyroiditis,bilateral nodules,goiter,nodule number distribution and benign thyroid nodules Malignancy was significantly correlated(P<0.05).Among the patients with malignant nodules,the level of PDW,the proportion of <55 years old,the level of BMI,the rate of diabetes,the rate of hyperlipidemia,the rate of nodule diameter ≤ 2cm,and the rate of thyroiditis were significantly higher than those in the benign group(P<0.05),while the multifocal rate,bilateral rate,and goiter rate were significantly lower than those in the benign group(P<0.05).There was no significant difference in NLR,PLR,RDW,gender,smoking,drinking,hypertension and benign and malignant thyroid nodules(P>0.05).2 NLR,PLR,PDW and RDW were not independent influencing factors of benign and malignant thyroid nodules.BMI,diameter grouping,bilateral or not,combined with goiter and number of nodules were the independent influencing factors of benign and malignant thyroid nodules(P<0.05).3 The AUC of NLR was 0.515,the sensitivity was 47.6%,and the specificity was 63.1%(P>0.05).The AUC of PLR was 0.538,the sensitivity was 45.0%,and the specificity was 64.2%(P>0.05).The AUC of PDW was 0.565,the sensitivity was equal to 41.0%,and the specificity was 70.4%(P<0.05).The AUC of RDW was 0.518,the sensitivity was 85.2%,and the specificity was20.1%(P>0.05).The AUC of the combination of NLR and PLR was 0.519,the sensitivity was 84.7%,and the specificity was 24.0%(P>0.05).The AUC of the combination of PDW and RDW was 0.571,the sensitivity was 68.6%,and the specificity was 44.7%(P<0.05).The AUC of the combination of NLR and PDW was 0.562,the sensitivity was 77.3%,and the specificity was 34.1%(P<0.05).The AUC of the combination of NLR and RDW was0.511,the sensitivity was 94.8%,and the specificity was 16.2%(P>0.05).The AUC of PLR combined with PDW was 0.566,the sensitivity was 49.8%,and the specificity was62.6%(P<0.05).The AUC of PLR combined with RDW was 0.548,the sensitivity was90.8%,and the specificity was 19%(P>0.05).The combined AUC of NLR,PLR,PDW,and RDW was 0.570,corresponding to a sensitivity of 91.3% and a specificity of 22.3%(P<0.05).Conclusions 1 There is a significant correlation between PDW and benign and malignant thyroid nodules,and the PDW level in patients with malignant thyroid nodules is significantly higher than that in patients with benign nodules.The routine serological index PDW may become a test index for predicting benign and malignant thyroid nodules;NLR,PLR,RDW had no significant correlation with benign and malignant thyroid nodules.2NLR,PLR,PDW,RDW are not the influencing factors of benign and malignant thyroid nodules.3 PDW,the combination of PDW and RDW,the combination of PDW and NLR,the combination of PDW and PLR,and the combination of PDW,NLR,PLR and RDW have certain diagnostic efficacy in predicting benign and malignant thyroid nodules,but further research is needed.Figure11;Table25;Reference 181... |
