Objectives In this study,middle-aged and elderly patients with severe psoriasis were selected as the research objects to evaluate the status of patients’ discharge readiness and anaiyze its influencing factors,for the future to carry out clinical hospital discharge guidance services,build guide planning and provides the basis for subsequent continuity targeted nursing intervention.Methods The convenience sampling method was adopted to select middle-aged and elderly patients with severe psoriasis who were hospitalized in the dermatology department of three Grade A hospitals in Chengde and Handan of Hebei Province from September2019 to December 2020 as research objects.After obtaining informed consent of 229 patients who met the inclusion and exclusion criteria.Investigators used the general data questionnaire,clinical treatment effect evaluation scale,discharge guidance quality scale,social support scale,discharge readiness scale,self-rating anxiety scale,self-rating depression scale to conduct face-to-face survey.Excel software was used to input all data into the database,SPSS 22.0 statistical software was used for statistical analysis,and the counting data were described by frequency and constituent ratio.The measurement data were described by x ±s,t-test was used for comparison between two groups,analysis of variance was used for comparison between multiple groups,and LSD-t test was used for further pial comparison.Multiple linear regression analysis was used for multivariate analysis and Amos 22.0 software was used for path analysis.Results 1 Status of discharge readiness in middle-aged and elderly patients with severe psoriasis: discharge readiness score was(91.30±11.42),which was in the middle level;There were 158 patients(68.99%)with intermediate and lower discharge readiness scores.Among the three dimensions of patients’ discharge readiness,the anticipatory support dimension scored low.2 Univariate analysis results show that: Discharge readiness score in age,marital status,educational level,family average per capita income,working status,reasons for leaving the hospital,admission and discharge hospitalization days,disease types,associated with other diseases types,treatment effect,anxiety,depression,quality of discharge teaching,social support was statistically significant difference(P < 0.05).3The results of multiple linear regression analysis showed that age,education level,discharge hospitalization days,associated with other diseases types,quality of discharge teaching,social support,anxiety and depression were the main influencing factors of discharge readiness in middle-aged and elderly patients with severe psoriasis.That is,patients with younger age,higher education level,more hospitalization days,fewer types of other diseases,higher level of social support,better quality of discharge guidance,and patients without anxiety and depression have higher level of discharge readiness.4 Path analysis results of influencing factors showed that education level,discharge hospitalization days,associated with other diseases types,anxiety and depression could directly or indirectly affect the patients’ discharge readiness.Age,quality of discharge teaching,social support are not only mediating variables,but also have direct effects on patients’ discharge readiness.Conclusions 1 The discharge readiness of middle-aged and elderly patients with severe psoriasis is at medium level,but the expected support of patients still needs to be further improved.2 Age,education level,discharge hospitalization days,associated with other diseases types,quality of discharge teaching,social support,anxiety and depression were the main influencing factors for discharge readiness of middle-aged and elderly patients with severe psoriasis.3 Age,the quality of discharge teaching,and social support can directly affect patients’ discharge readiness,or indirectly affect them as mediating factors.For elderly patients,we should strengthen the discharge education of patients and their families and improve the level of social support for patients,so as to help patients prepare for discharge and better return to their families and society.Figure4;Table17;Reference 137...
