Objectives Animal experiments: 1 to establish a scientific rat model of diabetic foot with transverse tibial bone transfer;2 to investigate the effects of different transverse transfer protocols on angiogenic factors in the serum of rats with diabetic foot;3 to preliminarily analyse the possible mechanisms of transverse tibial bone transfer in the treatment of diabetic foot;4 to study the vascular changes in the skin of lower limbs during transverse tibial bone transfer.Clinical trial: Design and application of a new minimally invasive osteotomy method to reduce surgical trauma and improve treatment outcomes in lateral tibial bone transfers.Methods Animal experiment: After 5 weeks of high-fat feeding,90 SD rats were set up as diabetic models,and the rats were randomly divided into 4 groups using the random number generator in SPSS: Group A(n=16): installation of the traverse outer frame but no traverse;Group B(n=16): installation of the traverse outer frame and one cycle of traverse;Group C(n=16): installation of the traverse frame,and then return to the traverse in the second traverse period;Group D(n=16): installation of the traverse frame and two traverse cycles.And 4 rats from each group were executed at the end of the traverse and consolidation periods of the 1st and 2nd traverse cycles respectively.Serum was collected to detect VEGF and PDGF,and HIF-1 and SDF-1 were detected in groups A and B.Results Animal experiments: 1 VEGF was significantly higher in group C than in group A at this time(P < 0.05);the rest were not significantly different,(P > 0.05).2 PDGF was significantly higher in group C than in group A at this time(P < 0.05);the rest were not significantly different,(P > 0.05).3 HIF-1 expression in group B was higher than that in group A at this time(P<0.05).4 SDF-1 expression in group A and B was not significantly different at this time(P>0.05).Clinical trial: Minimally invasive tibial bone relocation with preservation of blood flow to the relocated bone mass is an effective method of treating chronic ischaemic disease of the lower limb and can significantly reduce complications in the donor area.Conclusions Animal experiments: 1 The rat model of transverse tibial bone transfer for diabetic foot in this study is scientifically valid and provides a reference for further research on the mechanism of transverse tibial bone transfer for diabetic foot;2 transverse tibial bone transfer may regulate angiogenic factors such as VEGF and PDGF in blood after upregulating SDF-1 through HIF-1,which in turn promotes microcirculation in the lower limbs.3 Extending the maintenance period or repeating an extension cycle did not significantly increase the efficacy of tibial transverse bone transfer in the treatment of diabetic foot.Clinical trial: Minimally invasive tibial bone relocation with preservation of blood flow to the relocated bone mass is an effective method of treating chronic ischaemic disease of the lower limb and can significantly reduce complications in the donor area.Figure40;Table6;Reference109...
