Objective: 25% of patients with diabetes will suffer from ulcers and gangrene due to neuropathy,and microcirculation disorders due to arterial insufficiency caused by macrovascular and microvascular lesions,which is also called "diabetic foot(DF)".According to the International Diabetic Podiatry Working Group(IWGDF),diabetic podiatry is defined as a diabetic wound below the ankle that involves the whole layer of skin regardless of the course of the wound.Diabetic foot ulcers(DFU)are one of the important causes of disability and death in diabetic patients.Through the collection of cases were retrospectively analyzed,this study adopts lateral tibia bone removal treatment in patients with DFand simple debridement in combination with the VSD pressure suction two kinds of treatment efficacy analysis,for the clinical diabetic foot disease provide clinical treatment methods open train of thought and provides the theory basis for clinical decision making,At the same time,it also accumulated experience in the treatment of a wider range of other diabetes complications.Methods: The clinical efficacy and quality of life of 25 patients with diabetic foot treated with Ilizarov tension-stress law technique and 44 patients with diabetic foot treated with pure foot debridement combined with VSD negative pressure suction were collected and summarized.Among them,the indicators used in clinical efficacy analysis mainly included CTA grading of lower limbs diagnosed by imaging,TCSS score and wound healing.Quality of life was scored using a simplified SF-36 Health Survey Score Scale.Results: The 66 cases were completely followed up,and the rate of lost follow-up was 4.35 %.There was no statistical difference in the length of stay between the two treatments.Wound recovery: In the surgery group,the 17 patients had fresh and moist wound granulation tissue at 2 weeks after surgery and basically crusted wound at6 weeks after surgery,while in the conservative group,only 10 patients achieved good granulation growth within the same time,and the rest of the patients still needed debridement and dressing change.The CTA score of lower limbs in 12 patients in the surgery group was lower than before,and that in 2 patients in the conservative group was improved.Lower limb pain and low skin temperature were improved in 18follow-up patients in the surgery group,compared with the conservative group(P <0.05),the difference was statistically significant.The quality of life,after statistical analysis of follow-up data,(P < 0.05).Conclusion: The effect of Diabetic foot Lateral tibia migration operation group was better than that of non-operation treatment group.The length of hospitalization of the two patients is close.Tibial transverse bone transfer for diabetic foot is an acceptable treatment method with relatively accurate therapeutic effect.However,it should be pointed out that the patients with systemic condition can tolerate surgery and complete medical treatment.Simple debridement combined with VSD negative pressure suction technique has a weak therapeutic effect on diabetic foot compared with tibial transverse bone transfer technique.However,conservative treatment is relatively safe for patients with poor general condition who cannot tolerate surgery.
