Analysis Of The Postoperative Effect Of Poor Leukocyte Platelet-Rich Plasma On Rotator Cuff Repair

Objectives To evaluate the effects of different platelet concentrations of leucocyte platelet-rich plasma(LP-PRP)on postoperative pain,functional,and structural integrity(MRI)outcomes in patients who have undergone arthroscopic rotator cuff repair.Methods 80 patients were divided into control group(n = 40)and LP-PRP group(n = 40)by Random number table method,and then randomly divided into low concentration LPPRP group(n = 20)and high concentration LP-PRP group(n = 20).Evaluate Visual Analogue Scale(VAS)at 14 days,1 month,3 months,6 months and 12 months after operation were compared among all groups.Assess activity of flexion and abduction at 12 months postoperatively;American shoulder and Elbow Surgical Score(ASES),ConstantMurley score and University of California,Los Angeles score(UCLA)at 6 and 12 months after surgery;Sugaya grading 12 months after surgery.Results VAS scores at 14 days,1 month,3 months,6 months and 12 months after surgery in the high concentration LP-PRP group and ASES scores at 12 months after surgery were significantly better than those in the low concentration LP-PRP group(P<0.05).But it did not reach the minimum clinical significance change value(MCID).Neither concentration of LP-PRP reduced the retear rate(P>0.05).Compared with the control group,the outcome indexes of the low-concentration group at each time point of follow-up were not statistically significant.VAS(12m)scores and ASES(12m)were worse in patients with retears than in patients with successful healing(P<0.05),where VAS(12m)did not reach MCID,while ASES(12m)reached MCID.Conclusions The high concentration group had better partial clinical outcomes than the low concentration group,but the difference was not clinically significant.Low and high concentrations of LP-PRP can not improve postoperative mobility,pain,function,and retear rates of patients with small and medium rotator cuff tears.Therefore,this study does not recommend arthroscopic rotator cuff repair combined with LP-PRP for the treatment of rotator cuff injury.But more research will be needed to confirm this conclusion.Mid-and long-term loss of function in patients with retear was more prominent than shoulder pain.Figure1;Table12;Reference 151...
