Objective: To explore the short-term clinical effect of ultrasound-guided platelet rich plasma injection combined with exercise therapy on partial rotator cuff injury.Methods: from May 2020 to November 2020,60 patients with partial rotator cuff injury were randomly divided into PRP group and hormone group,30 cases in each group.In the PRP group,2ml platelet rich plasma was injected into the bursa of acromion group once under ultrasound guidance,and 2ml compound betamethasone and lidocari mixture was injected into the hormone group once.After the injection,exercise therapy was performed for 4 weeks.Shoulder flexion range of motion,shoulder abduction range of motion,VAS(visual analog scale)pain score,UCLA(University of California Los Angeles)score and constant(constant Murley score)score were evaluated before treatment,immediately after treatment,2 weeks after treatment and 4 weeks after treatment respectively.Results: 3 cases in PRP group and 2 cases in hormone group did not complete the final evaluation.Finally,27 cases in PRP group and 28 cases in hormone group were included in the statistical analysis.There were no serious adverse reactions in the two groups.Before treatment,there were no significant differences in general information,flexion range of motion,abduction range of motion,VAS score,UCLA score and Constant score between the two groups(P > 0.05).In PRP group,there was no significant difference in all evaluation indexes between before and immediately after treatment(P > 0.05);After 2 weeks and 4weeks of treatment,compared with before treatment,shoulder flexion activity,shoulder abduction range of motion,UCLA shoulder score and constant shoulder score were significantly improved,while VAS score was significantly decreased(P < 0.05).Compared with before treatment,the shoulder flexion range of motion,shoulder abduction range of motion,UCLA shoulder score and constant shoulder score of hormone group were significantly improved,while the VAS score was significantly decreased(P < 0.05).Immediately after treatment,although the hormone group was better than the PRP group(P <0.05),the difference between the two groups was not statistically significant at 2 weeks after treatment,and the indicators of the PRP group were better than the control group at 4 weeks after treatment,and the difference was statistically significant(P < 0.05).Conclusion: ultrasound guided injection of platelet rich plasma into the acromion bursa combined with exercise therapy or ultrasound-guided injection of hormone combined with exercise therapy can significantly reduce the pain of some patients with rotator cuff injury,improve the range of motion and shoulder joint function.The immediate effect of platelet rich plasma injection is not as good as that of hormone injection,but the short-term continuous improvement effect is better than that of hormone injection. |
