| Objective:This study carried out quality control circle group activities in a Grade-A tertiary hospital to reduce the rate of repuncture of arteriovenous fistula,analyze the causes of puncture failure of arteriovenous fistula,formulate corresponding nursing measures,reduce the damage to blood vessels caused by repuncture of arteriovenous fistula,and provide effective nursing strategies to reduce the incidence of complications of arteriovenous fistula and improve the quality of nursing services,to provide scientific theoretical basis and technical support for clinical medical staff.Methods:This study adopted literature review,quasi experimental research and questionnaire survey.Through reviewing the domestic and foreign literature,it was determined that reducing the repuncture rate of arteriovenous fistula in hemodialysis patients was the theme of this quality control circle activity.We selected 90 patients who met the research standard and used arteriovenous fistula for regular dialysis in the blood purification center of a Grade-A tertiary hospital in Kunming as the research objects.Before the implementation of the quality control circle(January to June 2021),the patients were in the control group,and after the implementation of the quality control circle activities(July to December 2021),the patients were in the observation group.The control group received routine operation and nursing care of arteriovenous fistula;on the basis of routine arteriovenous fistula operation and nursing care,the observation group conducted 6-month intervention in the activities of the quality control circle.The members of the quality control circle held regular group meetings every week to collate and summarize data,analyze and feed back the contents of the activities,and implement corrective nursing measures.The repuncture rate of arteriovenous fistula in the control group and the observation group was compared after the intervention of the quality control circle.At the same time,the pain degree of arteriovenous fistula puncture in the two groups was analyzed.The patient’s mastery of health education,incidence of complications of arteriovenous fistula and patient satisfaction.SPSS25.0 software was used for statistical analysis of the research data.The measurement data were expressed as mean±standard deviation((?)±s),and t test was used for comparison between the two groups;The counting data is expressed in(%),usingX~2Test,P<0.05 indicates that the difference is statistically significant.Results:After the intervention of quality control circle,the repuncture rate of arteriovenous fistula in the observation group decreased by 75.68%;The pain score of patients in the observation group for arteriovenous fistula puncture decreased from 2.5±0.23 to 2.0±0.17;the scores of hemodialysis related knowledge in the observation group increased by 45.08%;the complications of arteriovenous fistula(bleeding,infection,thrombosis,hematoma)in the observation group decreased,and the total incidence decreased by 78.57%;The patient satisfaction score of the observation group increased from 89.23±1.32 to 97.54±1.63.The difference between the two groups was statistically significant(P<0.05).Conclusion:Through the intervention of quality control circle,the repuncture rate of patients with arteriovenous fistula is significantly reduced,the pain degree of patients with arteriovenous fistula puncture is reduced,and the incidence of complications such as blood leakage,thrombosis and infection of arteriovenous fistula is reduced.It improves the mastery of hemodialysis related knowledge of patients with arteriovenous fistula;Improve patients’initiative and compliance.It improves the satisfaction of patients and their quality of life.The comprehensive quality and nursing quality of nursing staff were improved.The quality control circle activity is effective in reducing the repuncture of fistula in the care of arteriovenous fistula.The specific nursing measures in the activity can be used for reference,research and verification by the same industry. |
