BackgroundCarotid atherosclerosis(CAS)affects the blood supply to brain tissues,which may then affect people’s cognitive function,the blood uric acid(SUA)level,cholesterol,Factors such as TC and triglycerides(TG)are of great significance for the development of CAS and may be related to mild cognitive impairment(MCI),so it is possible to influence the progress of CAS and MCI.ObjectiveTo explore the relevant influencing factors of CAS induced MCI,so as to implement early intervention and reduce the occurrence and development of CAS induced MCI.MethodA total of 539 health screening cases were collected from Shangshui County People’s Hospital in Gupinghe Town from December 2021 to February 2022.Clinical data of the subjects were collected,including gender,age,height,weight,TC,TG,smoking history,drinking history,coronary heart disease history,diabetes history,homocysteine,etc.Hcy),SUA,CAS.The montreal Cognitive Functioning Scale(Mo CA)was used to assess cognitive functioning.According to the results of CAS,they were divided into normal control group and CAS group.Multivariate Logistic regression analysis was used to explore the influencing factors of CAS patients with MCI.The correlation between SUA level and Mo CA score was analyzed by Pearson correlation analysis.With carotid atherosclerosis combined with cognitive dysfunction as the dependent variable,the influence of carotid atherosclerosis combined with cognitive dysfunction was explored,and a clinical prediction model was built.Results(1)A total of 539 subjects were included and divided into two groups: normal control group(283 cases)and CAS group(256 cases).(2)Comparison of clinical data between the two groups showed no significant differences in age,BMI,TC,drinking history,diabetes history and Mo CA score(P > 0.05).(3)Comparison of clinical data between the two groups showed that there were significant differences in gender,education level,height,weight,TG,Hcy,SUA,smoking history and coronary heart disease history between the two groups(P<0.05).(4)Age was a risk factor for MCI in patients with atherosclerosis(OR=1.063,95%CI:1.023-1.104,P=0.001).(5)Elevated TG was a risk factor for MCI in patients with atherosclerosis(OR=1.346,95%CI:1.013-1.787,P=0.04).(6)Increased TC was a risk factor for MCI in patients with atherosclerosis(OR=1.406,95%CI:1.112-1.779,P=0.004).(7)Elevated Tcy was a protective factor for MCI in patients with atherosclerosis(OR=0.939,95%CI:0.896-0.984,P=0.009).(8)Increased SUA is a risk factor for MCI in patients with atherosclerosis(OR= 1.005,95%CI:1-1.01,P=0.031)(9)In CAS combined with MCI,SUA level was negatively correlated with Mo CA score(r=0.-0.01025,P<0.05).(10)The ROC curve of the model was constructed by using the prediction probability and whether CAS had cognitive abnormalities,and the consistency of the model was measured.It was found that the ROC area AUC was 0.804,indicating good consistency of the model.The actual curve of the model was consistent with the trend of Bayesian calibration curve and ideal curve,indicating that the actual prediction ability of the model was relatively ideal and strong.ConclusionSUA,TG,TC,age and Hcy are related to the occurrence of CAS induced MCI,and may be risk factors for CAS induced MCI,and Hcy may be protective factors for CAS induced MCI...
