BackgrouedThe prevalence of knee osteoarthritis is increasing year by year.Because pain seriously affects the quality of life of patients,at present,in addition to joint replacement,conservative treatment by drugs alone is not effective.Saphenous nerve block therapy has few side effects,and can relieve pain of patients’ peripheral joints,improve the therapeutic effect and quality of life.Therefore,it is necessary to explore the most suitable treatment plan of saphenous nerve block.ObjectiveTo study the therapeutic effect of different concentrations of ropivacaine hydrochloride combined with intraarticular injection of sodium hyaluronate on patients with knee osteoarthritis.MethodsRandomly divided 144 knee osteoarthritis patients into low concentration groups(Group L),high concentration groups(Group H)and control group(Group N).48 cases in each group.Group L received an intra-articular injection of sodium hyaluronate combined with 0.25 % ropivacaine cryptogenic nerve block;Group H received an intra-articular injection of sodium hyaluronate combined with 0.5 % ropivacaine cryptogenic nerve block;Group N received an intra-articular injection of sodium hyaluronate without ropivacaine cryptogenic nerve block.Observing and recording the treatment effects before,4 weeks after,and 12 weeks after treatment based on WOMAC(The Western Ontario and Mc Master Universities Arthritis Index)and the adverse reactions(such as local anesthetic poisoning,skin bruising,local hematoma,infection).ResultsIntra-group comparison: The WOMAC scores of N,L and H groups at 4 weeks(96.33±21.73,85.27±21.70,81.58±20.76)and 12 weeks(90.27±23.55,73.91±23.60,67.31±20.68)were lower than those before treatment(P<0.05).In L and H groups with cryptogenic nerve block,the WOMAC scores decreased at 12 weeks after treatment compared to 4 weeks(-11.36±6.35,-14.27±8.19)(P<0.05);There was no significant difference in WOMAC scores between the control group at week 12 and week 4 after treatment(-6.06±1.82).inter-group comparison: after treatment,the WOMAC scores of the two groups with cryptogenic nerve block were lower than group N(P<0.05),but there was no difference between the two groups with cryptogenic nerve block(P>0.05).In addition,group L(6)had fewer adverse effects than group H(15)(P<0.05).ConclusionBoth low-concentration and high-concentration intraarticular injections of ropivacaine combined with sodium hyaluronate were able to reduce WOMAC scores and significantly improve quality of life in patients with knee osteoarthritis.
