Collation And Research Of The Academic Experience Of Shen Jinao,a Famous Doctor In Qing Dynasty,in The Treatment Of Hepatobiliary Disease

Objective:In this paper,the famous doctor Shen Jinao’s works Shen Shi Zunsheng Book and Shen Qianlu Medical Case as the research object,systematically sorted out his medical theories and medical cases for the treatment of hepatobiliary diseases,summarized his medication rules and academic experience for the treatment of hepatobiliary diseases,and enriched the academic content of TCM treatment of hepatobiliary diseases.Methods:This paper comprehensively adopts the methods of literature research and data mining,and takes the works of Shen Shi Zunsheng Shu and Shen Qianlu Medical Case written by Shen Jinao,a famous doctor in the Qing Dynasty,as the main research objects,to summarize his medication rules and academic experience in the treatment of liver-biliary diseases.Results:First of all,by Shen Jinao discrimination treatment of jaundice,accumulation,swelling,pain,gall disease,malaria and other diseases experience research found that:(1)Shen Jinao as a southern doctor likes to use "poulter" the word to describe jaundice.In terms of treatment,Shen Jinao proposed the principle of partial treatment and phased treatment.He preferred to use ganping instead of cold and cold,and identified the etiology and prognosis from body pain,urination and thirst,which was helpful to distinguish the etiology and send prescription drugs.In addition,Shen Jinao had the courage to innovate the combined treatment of liver and kidney,and created Shen’s icterus prescription to treat icterus.(2)Shen Jinao advocated focusing on the treatment of accumulation,proposed to focus on the regulation of liver qi,and insisted on the idea of nourishing the positive and consolidating the root of accumulation.He also proposed many external treatment methods such as holding medicine and promoting accumulation and guiding the work.He also created Shen’s Cotton seed pills,Shen’s blood pressure pills and other prescriptions for the treatment of accumulation,which provided more TCM characteristic therapies for clinical diagnosis and treatment of accumulation.(3)Shen Jinao believed that the root of distension was the spleen,and made a detailed identification of distension,induced distension and single abdominal distension.Shen Jinao advocated to treat distension mainly by tonifying and eliminating accumulation and making good use of prescriptions such as Tiaozhong Jianpi pills.In addition,Shen Jinao added the contents about the life precaution of distention,and suggested that the disease tendency could be distinguished by the trend of distention in terms of prognosis.(4)Shen Jinao believed that hypochondriac pain was hepatic meridian disease,and deeply analyzed the syndrome differentiation and treatment of hypochondriac pain from the perspectives of left and right,qi and blood,deficiency and deficiency,internal and external,etc.,and advocated the combination of internal administration and external application.(5)Shen Jinao divided gall disease into 11 types,advocated the harmonization of the five viscera organs,attack and tonifying,and created Shen’s gall capsule pills to reduce phlegm,weaken and strengthen,eliminate gall and dissipate knot.(6)Shen Jinao believed that malaria was divided into 32 kinds,and gave priority to the diagnosis of sweating in patients.The principle of treatment was to dissipate the evil and maintain good health,and the blind use of buchanhu was opposed.He also recognized the harm caused by the mistreatment of malaria,and stressed the importance of observing the characteristics of the disease,so as to prevent the disease before it broke out and prevent the disease after it broke out.Secondly,the TCM inheritance computing platform V3.0 was used to analyze the basic information of prescriptions,prescription rules and the core prescriptions of evolution,taking Shen Jinao’s prescriptions for treating liver-biliary diseases as the research object.After screening,a total of 327 prescriptions were included,involving 308 flavors of drugs,with a total frequency of 2632 drugs.The medicinal properties are mainly warm,cold and flat;The taste of medicine is mainly bitter,sweet and sweet.Return to the spleen,stomach,lung mainly;The main effect is tonifying deficiency,regulating qi and clearing heat.There were 25 traditional Chinese medicines with frequency ≥30,among which tangerine peel was the first.There were 8groups of high frequency drug combination,involving 8 drugs;Association rule analysis obtained 9 groups of drug combinations,involving 7 drugs;The drug relationship diagram showed that the addition and reduction formula of Bupi Wan composed of poria,Bai Zhu and Tangerine peel was the basic prescription for the treatment of hepatobiliary diseases.The analysis of association rules showed that Bupi Wan,Erchen Tang and Liujunzi Tang played a more important role in the treatment of hepatobiliary diseases.Conclusion(s):Through the analysis of traditional literature review and modern information processing technology,it was found that Shen Jinao had the following characteristics in discriminating the treatment of hepatobiliary diseases:(1)Shen Jinao was opposed to "no replenishment of liver",and proposed that liver depression,liver reverse and other liver diseases were caused by liver insufficiency.Therefore,in terms of treatment,Shen Jinao advocated the prudent use of attack and attack,and the proper use of nourishing liver blood and warming liver qi.(2)Shen Jinao believed that liver disease was often reversed,and emphasis should be placed on soothing the liver and relieving depression.(4)Shen Jinao believed that liver deficiency,liver depression and liver inversion could cause liver fire exuberant,so he liked to use cooling liver blood.(4)Shen Jinao stressed that "liver harmony does not steal soil" and advocated the same treatment of liver and spleen.On the basis of inheriting the sages,Shen Jinao put forward his unique ideas on the differentiation and treatment of liver diseases from a new Angle.
