ObjectiveThis article focuses on the treatment of VaD by Zhongfeng Qixue Decoction,from two aspects of clinical observation and animal experiment.The clinical observation part was based on the theory of "Qixu Xueyu" of traditional Chinese medicine to observe the clinical effect of Zhongfeng Qixue Decoction in the treatment of VaD.In the animal experiment,the Reperfusion combined with bilateral common carotid artery ligation method was used to establish a VaD rat model.The intervention effect of Zhongfeng Qixue Decoction on VaD model rats and its possible mechanism were analyzed by behavioral and pathological methods.Methods1.Sixty-two VaD patients in our hospital who met the criteria of this study were included and randomly divided into western medicine control group and Chinese medicine treatment group.Subjects included in the western medicine control group were treated with Donepezil hydrochloride(Aricept)on the basis of conventional treatment,and subjects in the Chinese medicine treatment group were treated with Zhongfeng Qixue Decoction on the basis of the Western medicine control group.Subjects in both groups underwent 4-week intervention.SDSVD,MMSE and ADL scores were performed on subjects in both groups before and after treatment,and the total clinical efficacy rate and the score changes of the above score scale were compared between the two groups.2.Animal experiments: In this study,49 healthy SPF SD rats were selected and randomly divided into sham operation group,model group,Chinese medicine treatment group and western medicine control group,with8 rats in each group(after excluding modeling failure and death of rats).The VaD rat model was established by reperfusion combined with bilateral common carotid artery ligation,and Morris water maze was used to confirm the success of modeling.After successful modeling,the rats in the traditional Chinese medicine group and the western medicine control group were given gastric administration of Zhongfeng Qixue Decoction and Donepezil hydrochloride,respectively.The rats in the sham operation group and the model group were given gastric administration of normal saline at the same volume,once a day,for a total of 4 weeks.Morris Water Maze was used again the next day after the intervention to conduct behavioral tests,and the spatial learning and memory abilities of each group were compared,especially the improvement of cognitive function of VaD rats after medication intervention.The day after the Morris water maze experiment was completed,the rats were killed and the hippocampal tissues were taken.RT-q PCR was used to detect and compare the changes of miR-9 and miR-124 in the hippocampal tissues of the rats in each group,and the data of the results and the Morris water maze behavioral test results were collated and analyzed,finally drawing a conclusion.Results1.Clinical research results: There were no significant differences in SDSVD,MMSE and ADL scores between the two groups of VaD subjects before treatment(P>0.05).After 4 weeks of treatment,the SDSVD score after treatment was lower than that before treatment(P<0.05),and the MMSE and ADL scores after treatment were higher than those before treatment(P<0.05),and the differences were statistically significant.The SDSVD scores of the two groups decreased(P<0.05)and the ADL scores increased(P<0.05)after treatment,with statistically significant differences between the two groups.The MMSE scores of the two groups were improved after treatment(P>0.05),but there was no statistically significant difference.2.Animal experiment results:(1)Compared with the sham operation group,(1)In terms of behavioral detection,the escape latency of the model group was prolonged(P<0.05),the time of first crossing the platform was prolonged(P<0.05),the time of staying in the target quadrant was shortened(P<0.05),and the number of crossing the platform was reduced(P<0.05).(2)In terms of RT-q PCR results,the expression levels of miR-9 and miR-124 in the hippocampus of the model group were significantly increased(P<0.05).All the above were statistically significant.(2)Compared with the model group,(1)In terms of behavioral test,the escape latency and the first time to cross the platform in the TCM treatment group and the western medicine control group were shortened(P<0.05);The time spent in the target quadrant was prolonged(P<0.05),and the number of crossing the platform was increased(P<0.05).(2)In terms of RT-q PCR results,the expression levels of miR-9 and miR-124 in the hippocampus of the Chinese medicine treatment group and the western medicine control group were significantly decreased(P<0.05).All the above were statistically significant.(3)Compared with the TCM treatment group:(1)There was no significant difference in the escape latency between the western medicine control group and the TCM treatment group from the 1st to the 4th day(P>0.05).On the 5th day,the escape latency of the western medicine control group was longer than that of the Chinese medicine treatment group(P<0.05).There was no significant difference in the time of first crossing the platform and the time of staying in the target quadrant between the two groups(P>0.05),which was not statistically significant.The number of crossing the platform in the western medicine control group was significantly reduced(P<0.05).(2)In terms of RT-q PCR results,there was no significant difference in the expression of miR-9 and miR-124 in the hippocampus between the two groups(P>0.05).Conclusions1.Clinical studies have found that Zhongfeng Qixue Decoction in the treatment of VaD can improve the degree of cognitive dysfunction and the quality of life of patients,and the clinical efficacy of VaD patients is generally satisfactory,without obvious toxic and side effects.2.In the animal experiments,it was found that the spatial learning and memory of VaD rats were impaired,and the expression levels of miR-9 and miR-124 in the hippocampus were increased.After the treatment with the Zhongfeng Qixue Decoction,the spatial learning and memory disorders of VaD rats were improved,and the expression levels of miR-9 and miR-124 in the hippocampus were decreased.The abnormal expression of miR-9 and miR-124 may be one of the pathogenesis of VaD.3.The treatment of VaD by Zhongfeng Qixue Decoction circulation may improve the symptoms of cognitive impairment in VaD rats by inhibiting the abnormal expression of miR-9 and miR-124. |
