| Objective: This study aims to screen 50 ancient TCM books which is related to the diagnosis and treatment of hidrosis syndrome,and sorts out the pathogeny,pathogenesis,symptoms,prescriptions and prognosis of hidrosis syndrome in the ancient TCM books to establish a thematic database of ancient traditional Chinese medicine books of hidrosis syndrome,and analyzes the database of ancient TCM books of hidrosis syndrome based on data mining,summarizes the rules and characteristics of diagnosis and treatment of hidrosis syndrome in ancient traditional Chinese medicine books to construct the TCM diagnosis and treatment auxiliary decision-making scheme of hidrosis syndrome,and performs a clinical consistency evaluation by collecting 100 clinical cases from the real-world of hidrosis syndrome to organize out a decision plan of TCM diagnosis and treatment of hidrosis syndrome more in line with clinical needs,and provides reference for the clinical diagnosis and treatment and research of hidrosis syndrome.Methods: The 50 ancient TCM books relate to the hidrosis syndrome of diagnosis and treatment are searched for in the system of "combine treatment of disease,pulse and syndrome" by using the keywords of "sweat","hidrosis syndrome","spontaneous perspiration" and "night sweat".The name overview,etiology,pathogenesis,symptoms,and therapeutic medication of hidrosis syndrome are organized and mined through Microsoft Excel to establish a database of ancient traditional Chinese medicine books of treatment of hidrosis syndrome.The study made frequency number analysis,association analysis,and cluster analysis of the database of ancient TCM books of treatment of hidrosis syndrome by using the statistical software,summarized the rules and characteristics of diagnosis and treatment of hidrosis syndrome,and constructed the first draft of TCM diagnosis and treatment auxiliary decision-making scheme of hidrosis syndrome.The Delphi method was used to collect the opinions of clinical experts in TCM internal medicine.The expert discussion meeting was hold.The final draft was formed after modification.And the final draft is evaluated consistently based on real world clinical cases.Results:(1)Data mining:(1)Frequency analysis: Among the 50 kinds of ancient TCM books,the etiology of hidrosis syndrome mainly recorde 6categories: the outer evil invasion,excessive desire for labor,emotional internal injury,excessive drinking and food,body deficiency after illness and therapeutic error.There are 20 kinds of common pathogenesis,among which the frequency of Yin deficiency,fire excess,Yang deficiency,heat evil forcing steam,surface deficiency and qi deficiency are more than 19 times.There are 30 kinds of common symptoms,among which spontaneous sweating,night sweating,head sweating,excessive sweating,fever,unfavorable urination,poor appetite and delirium are more than 20 times.There are 10 kinds of commonly used treatment methods,among which the frequency of regulating Yingwei,supplementing qi and strengthening surface,purging heat and attacking inside,nourishing Yin and lowering fire,clearing heat and diuresis,and expelling and purging heat are more than 8 times.Among the common decoctions,13 decoctions are more than 10 times,and the top 3 were Danggui Liuhuang Decoction,Guizhi Decoction,Xiao Chaihu Decoction.15 common Chinese medicines are higher than 80 times,the first top 3 flavors are astragalus,ginseng,licorice.The top 5 types of highest frequency of TCM were medicine of filling the weakness,medicine of relieving exterior syndrome,heat-clearing medicine,astringent medicine,medicine of eliminating dampness.(2)Association analysis: In the symptom to symptom association analysis of the 50 ancient TCM books of hidrosis syndrome,the minimum support is set of 2%,the minimum confidence is set of 20% and the maximum number of preceding term is set of 5 and 49 symptom item sets of hidrosis syndrome are obtained.In the association analysis of Chinese medicine to Chinese medicine,the minimum support is set of 10%,the minimum confidence is set of 50%,the maximum number of preceding term is set of 5 and 59 Chinese medicine item sets are obtained.(3)Cluster analysis:Symptom data with frequency greater than 10 are imported into SPSS Statistics 26.0,and 8 groups of syndrome of hidrosis syndrome combinations are obtained.Data of TCM with a frequency greater than 5 are imported into SPSS Statistics 26.0,and 13 TCM combinations of research significance are obtained through analysis.(2)Questionnaire results: Based on the results of data mining results,the first draft of the decision plan of TCM diagnosis and treatment assistance based on 50 ancient books of traditional Chinese medicine was formulated together with the first round of expert questionnaire,and the second round of expert questionnaire was revised according to the results of the first round of expert questionnaire.The positive coefficient of experts in the first and second round of questionnaires was 83.33% and83.33%,respectively.On this basis,by combining with the results of two rounds of questionnaire survey,the first draft of the program was revised,and the final draft of TCM diagnosis and treatment auxiliary decision program of 50 ancient books of traditional Chinese medicine was formed.And the draft was passed by the expert argumentation meeting.(3)Conformance evaluation:In the first affiliated hospital of Guangxi University of Traditional Chinese Medicine,100 clinical cases were collected for the final version of the protocol,and the diagnosis,diagnostic basis,type of syndrome classification,type of syndrome diagnosis basis,rules of treatment,main prescription,and medicine were 100%,100%,85%,93%,77%,71% and 80% respectively.Conclusions: The diagnosis and treatment of ancient TCM books of hidrosis syndrome have a rich academic experience.This study is based on 50 ancient books of traditional Chinese medicine,summarizes the rules and characteristics of the diagnosis and treatment of hidrosis syndrome in ancient medical books,forms the TCM diagnosis and treatment auxiliary decision-making scheme,collectes the revised opinions after two rounds of clinical expert questionnaire survey and conductes the consistency evaluation based on the real world which makes this scheme more scientific,rigorous and objective.It promotes the two-way interaction between the diagnosis and treatment experience of hidrosis syndrome and TCM in ancient TCM books,makes the protocol better serve the clinic,provides reference for clinicians to treat hidrosis syndrome and TCM syndrome differentiation,and realizes the value of ancient traditional Chinese medicine books.It also provides reference and ideas for hidrosis syndrome and TCM clinical diagnosis,treatment and related research. |
