Objective:By comparing and observing the changes of scoring indexes in different time periods before and after treatment, the mechanism and effect of laurel tendon manipulation combined with Chinese medicine scalding therapy on lumbar disc herniation were discussed. And to explore the advantages and disadvantages of Lijin manipulation in the treatment of lumbar disc herniation,to provide effective data for the selection of indications for manipulative treatment of LDH,and further enrich the connotation of spinal orthopedic external treatment.Methods:1.This study aimed to collect 94 eligible patients with the lumbar disc herniation aged 18~65 years old from the second ward of the Spine and Orthopedics Department of Liuzhou Traditional Chinese Medicine Hospital from January~September 2022,and randomly divided them into experimental group and control group, with 47 cases in each group,so as to facilitate comparative research.2.Research method:The control group was treated by conventional Tuina manipulation, and the treatment plan was 5 consecutive days and 2 days of rest was a treatment course, for a total of 3 weeks. The treatment plan of the experimental group was 1 time in 3 days, each treatment time was about 30~40min,3 times for 1 treatment course,a total of 3 courses of treatment, and the duration was 3 weeks.3.Observation indicators:The VAS scores and JOA scores of patients in the two groups were recorded before treatment, the first course of treatment and the last course of treatment.Follow-up was conducted 1 month and 3 months after treatment, respectively, and the clinical efficacy of the two groups was evaluated.Finally,SPSS22.0 software was used to statistically analyze the VAS score and JOA score data collected in each period of the two groups, and the conclusion was drawn.Results:1.Before treatment, general data(including gender, age and course of disease)were compared between the two groups, and no significant differences were found(P>0.05),indicating comparability. In addition, before treatment, there were no significant differences in VAS scores and JOA scores between the two groups(P>0.05).2.Comparison of VAS scores:There was no significant difference in VAS scores between the two groups before treatment and the first course of treatment(P>0.05);There was a statistically significant difference in VAS scores between the two groups in the last course of treatment(P<0.05),indicating that the experimental group could quickly and effectively relieve low back and leg pain and limited mobility. In the comparison of 1 month and 3 months after follow-up treatment, the VAS scores of the two groups were significantly different(P<0.05),indicating that the treatment effect of the experimental group and the control group had a certain continuity, and the VAS score of the experimental group decreased significantly compared with the control group, indicating that with the extension of the treatment time, the experimental group was better than the control group in reducing pain and relieving symptoms.3.Comparison of JOA score:In the comparison of JOA score between the experimental group and the control group before treatment,no significant difference was found(P>0.05).However,in the first and last course of treatment after treatment, there was a significant difference in the JOA score between the experimental group and the control group(P<0.05),indicating that the experimental group could effectively reduce the pain of the lower back and legs and optimize the mobility function of the lumbar spine. In the comparison between the two groups 1 month and 3 months after follow-up treatment, the JOA score of both groups increased, but the experimental group increased significantly compared with the control group, and the difference was statistically significant(P<0.05),indicating that after treatment, the efficacy of both groups had a certain sustained effect, which also indicated that the experimental group and the control group had different degrees of improvement in the patient’s lumbar function and mobility, but the efficacy and improvement of Gui Pai Li tendon manoeuvre combined with Chinese medicine ironing in the treatment of patients with lumbar disc herniation were better than conventional Tuina manipulation.4.Clinical efficacy determination:The experimental results showed that the treatment effect of the experimental group was better than that of the control group. There were 19 obvious cases,24 effective cases and 4 ineffective cases in the experimental group, with a total effective rate of 91.49%.In the control group, there were 8 effective cases,27 effective cases and 12 ineffective cases, and the total effective rate was only 74.47%.According to the statistical analysis, the significant difference between the two groups was statistically significant(P<0.05),which proved that the curative effect of GUI Pai Lijin technique combined with Chinese medicine ironing was real and effective.Conclusion(s):1.By comparing the basic data of gender, age and course of disease between the two groups, the results showed no statistical difference(P>0.05),indicating that the two groups were comparable.2.GUI Pai Lijin manipulation combined with Chinese medicine ironing treatment can quickly and effectively relieve the discomfort caused by lumbar disc herniation, improve the patient’s lumbar functional range of motion and pain.3.The application of GUI Pai Lijin manipulation combined with Chinese medicine ironing in the treatment of lumbar disc herniation has significant clinical effect, which can further reduce the patients’ VAS score, improve the JOA score, reduce the patients’ back and leg pain, enhance the patients’ lumbar functional activities, and improve the patients’ quality of life.4.Safety status:During the whole treatment process,no adverse events and no shedding or eliminating cases occurred in the two groups, and the vital signs of the two groups were stable before and after treatment and during follow-up.
