| Objective: to analyze the effect of uric acid on platelet function and arteriosclerosis in patients with essential hypertension,and to observe the clinical efficacy of Santeng Yangxin Tongmai Recipe on patients with hypertension of “Qi Xu Xue Yu”type,and to evaluate whether uric acid is an important indicator of platelet activation and arteriosclerosis in hypertension.Above all,the ultimate objective is to summarize the safe and effective treatment of hypertension with hyperuricemia,and provide theoretical basis and methods for the traditional Chinese medicine treatment and western therapy.Methods: First,From March 2020 to Febrary 2021,outpatients or inpatients of Division 1 of cardiovascular department of The First Affiliated Hospital of Guangxi University of traditional Chinese Medicine were selected in our clinical experiment,including 30 patients with essential hypertension(EH),and 30 patients with hypertension with hyperuricemia.The general data(name,gender,age,height,weight,smoking history,drinking history,family history)were collected,and the serum uric acid,PAC-1,CD62 p,PLT,MPV,PCT,PDW,ba PWV,ABI,arteriosclerosis index,morning systolic blood pressure,morning diastolic blood pressure,liver function,renal function and other indicators were observed,and the correlation between uric acid and related indicators was analyzed.Second,60 patients with hypertension with hyperuricemia whose TCM Syndrome differentiation is “Qi-deficiency and Blood-stag” in the First Affiliated Hospital of Guangxi University of traditional Chinese medicine from March 2020 to Febrary 2021 were selected and divided into the observation group and the control group.30 patients in the control group were treated with “amlodipine besylate tablets 5mg qd” to control their blood pressure.Besides,thier lifestyle intervention included low salt,low fat and low purine diet,regular work and rest,and appropriate aerobic exercise.30 cases in the observation group were treated with Santeng Yangxin Tongmai prescription in addition to the basic treatment of the control group,“50ml bid”for one month.The changes of related indicators including serum uric acid,PAC-1,CD62 P,ba PWV,ABI,morning systolic blood pressure,morning diastolic blood pressure,liver and kidney function were observed before and after treatment in the two groups.The TCM syndrome score and adverse reactions were compared,and the differences after treatment were compared.SPSS 22.0 was used to analyze the data,with P <0.05 as statistically significant.Result: First,Comparison of clinical data of patients with essential hypertension and hypertension with hyperuricemia: there was no significant difference in general data between the two groups(P > 0.05);UA,CD62 P,PAC-1,ba PWV,ABI,morning systolic blood pressure and morning diastolic blood pressure between the two groups were statistically significant(P < 0.05).Second,Univariate correlation analysis showed that uric acid was correlated with PAC-1,CD62 p,ba PWV,ABI,PCT,PLT,morning systolic blood pressure and morning diastolic blood pressure(P < 0.05).Third,Patients with hypertension and hyperuricemia were divided into control group and experimental group.Before treatment,the general data of the two groups had no statistical significance(P > 0.05);After the intervention,there were significant differences in blood pressure,uric acid,CD62 P,morning blood pressure and TCM symptom scores between the control group and the experimental group(P < 0.05);PAC-1 in the experimental group before and after treatment had significant difference(P < 0.05),but there was no significant difference in the control group(P > 0.05);ba PWV,ABI,Cre,AST,ALT in the control group and the experimental group before and after treatment had no significant difference(P > 0.05).After treatment,the uric acid,systolic blood pressure,diastolic blood pressure and morning systolic blood pressure between the two groups were statistically significant(P < 0.05).After treatment,the morning diastolic blood pressure,ABI,PWV,SCR,AST and ALT of the two groups were not statistically significant(P > 0.05).Conclusions: High uric acid mediates platelet activation and arteriosclerosis in hypertension,which is a risk factor of vascular injury and should be paid attention to.Santeng Yangxin Tongmai prescription can effectively control the blood pressure of patients with “Qi-deficiency and Blood-stag” type hypertension with hyperuricemia,reduce platelet activation,and reduce the morning blood pressure of patients,and has good safety.There was no significant change of PWV and ABI after treatment.Considering that arteriosclerosis is a long process in hypertension,Santeng Yangxin Tongmai prescription can not improve arteriosclerosis in a short time. |
