| Objective: In order to better explore and inherit the academic thought of Professor Luo Weisheng in the treatment of chronic hepatitis B,through sorting out typical cases and combining with data mining technology,the application and analysis of Professor Luo Weisheng’s drug rules in the treatment of chronic hepatitis B,inherit the clinical experience of the tutor and provide clinical reference for the study of syndrome differentiation and treatment of chronic hepatitis B.Methods:(1)Select patients with chronic hepatitis B who meet the criteria according to the requirements through the electronic case system of the hospital outpatient department.Use Excel table to establish general data table according to the number,age,gender,TCM diseases,syndrome types and TCM composition.Use the ancient and modern cloud medical record platform to make statistics on the frequency of gender,age,syndrome types and TCM,analyze association rules and systematic clustering.(2)In the form of outpatient consultation,typical cases were copied and recorded,and Professor Luo Weisheng’s academic thoughts and clinical experience in the treatment of chronic liver disease,especially chronic hepatitis B related diseases,were summarized in combination with his unique insights and own understanding of the disease in the process of follow-up.Results: 1.The characteristics of traditional Chinese medicine treatment of chronic hepatitis B by Professor Luo Weishengare as follows:(1)TCM diseases and syndrome types: the diagnosis of traditional Chinese medicine is mainly liver syndrome,silliness and strain,and hypocholic pain,accounting for 40.96%,17.70%,15.04%,respectively;the syndrome types are mainly liver qi and spleen deficiency,liver biliary dampness and heat,and liver and kidney Yin deficiency,accounting for 22.12%,18.58%,13.27%,respectively.(2)Frequency of traditional Chinese medicine: A total of 167 kinds of traditional Chinese medicine,the cumulative total frequency of drug use reached 2192 times,and 40 kinds of traditional Chinese medicine with drug frequency ≥15.(3)Analysis of the four Qi of drugs:mainly warm(26.51%),slightly cold(20.76%),flat(20.67%)three characteristics.(4)Analysis of five tastes of drugs: sweet(49.86%),bitter(44.25%),tangential(34.53%).(5)Drug channel analysis: mainly spleen(50.64%),liver(44.94%),lung(34.63%),kidney(31.25%),stomach(28.38%),heart channel(20.39%).(6)Drug clustering analysis: there are 3herbal clustering groups and several drug combinations appear,Mo Dried Lotus-Rhubarb-Lychee Kernel-Wu Wei Zi(protect the liver and lower enzymes),Yin Chen-Malt-Tian Ji Huang – Ji Gu Cao-Che Qian Cao(dispel dampness and promote bile),Dan Shen-Red Peony-Turtle Nail-Tao Ren(invigorate blood circulation and resolve blood stasis),Ze Di-Hou Pu-Coix Seed-Han Xia(promote dampness and resolve turbidity),Sheng Di Huang-Mai Dong-Bei Sha Shen-Dang Shen(tonifying Qi and nourishing Yin),Chen Pi-Fu Ling-Bai Shu-Chai Hu-Bai Shao(draining the liver and strengthening the spleen),Huang Qi-Angelica Sinensis-Shu Di Huang-Chuan Xiong(tonifying Qi and Blood),Fructus Lycii-Wu Zhu Yu-Cornus Officinalis-Shan Yao-Radix et Rhizoma Ligustici(tonifying the liver and kidney),Citrus Aurantium-Hou Pu(eliminating distension and fullness),Gardenia-Wu Yao(clearing heat and relieving pain).(7)Analysis of drug association rules: a total of 14 groups of associated drugs,the main core drug groups are "Atractylodes → Poria","Atractylodes → Chai Hu","Poria → Chai Hu","Chai Hu → Paeonia lactiflora","Paeonia lactiflora→ Paeonia lactiflora"."Bai Hu → Bai Shao","Bai Hu → Chen Pi","Bai Shao → Fu Ling","Bai Zhu → Chen Pi","Bai Shao → Bai Zhu","Bai Shao → Bai Zhu"."Bai Shao → Bai Zhu","Bai Shao → Chen Pi".2.Professor Luo Weisheng’s academic thoughts on the treatment of chronic hepatitis B: Professor Luo Weisheng believes that chronic hepatitis B takes liver depression and spleen deficiency as the basic pathogenesis and liver as the main disease site,involving gallbladder,spleen,kidney and stomach;The course of its disease is characterized by liver loss and drainage,bile gas overflow;Liver disease and spleen and stomach,spleen deficiency and dampness,disharmony between liver and stomach,chronic disease and kidney,spleen and kidney deficiency.In terms of treatment,Yinchenhao Tang is the representative prescription of clearing dampness-heat,Si Ni San is used to soothe the liver,strengthen the spleen and nourish the blood,and Si Wu Tang is used to supplement the liver,spleen and kidney.The main functions of commonly used drugs are soothing the liver and invigorating the spleen,removing dampness and removing yellow,promoting blood circulation and removing blood stasis,tonifying the liver and kidney,and invigorating qi and activating blood circulation,so as to remove dampness,remove blood stasis,regulate qi and invigorate blood circulation,and make the veins accessible.Conclusions: 1.Professor Luo Weisheng treats chronic hepatitis B with the following prescriptions:(1)The four qi are dominated by warm,slightly cold,and flat drugs;the five flavors are mainly sweet,bitter,and pungent;and the main channels are the spleen,liver,lung,kidney,and stomach channels;the commonly used prescriptions are Yinchenhao Tang,Si Ni San,and Si Wu Tang;(2)The treatment idea is to "dredge the liver,benefit the gallbladder,and nourish the spleen and kidney.2.Professor Luo Weisheng’s academic thoughts on the treatment of chronic hepatitis B: chronic hepatitis B is mainly caused by positive deficiency and evil substance,and improper emotional diet and treatment are the causes;Pathogenic factors,dampness-heat and blood stasis were the main pathological factors.The disease is divided into three stages,the early stage of the disease in the liver and gallbladder,the middle stage of the liver and spleen,the late stage of the disease related to the heart and kidney;The main treatment is based on the principle of "convergence of East and West and joint intervention",and the main treatment is to treat the liver and the spleen,to benefit the liver and kidney,and to benefit the qi and blood. |
