| ObjectiveAiming at the deficiencies in the establishment and application of the medical record database,the database of chronic heart failure of modern famous traditional Chinese medicine was constructed by improving the data processing and organization form of the database.Data mining was conducted for the medical cases included in the database of chronic heart failure medical cases of modern famous traditional Chinese medicine to summarize the ideas of syndrome differentiation and treatment of chronic heart failure and the law of medication.MethodThe effective medical records of the treatment of chronic heart failure were obtained from literature database and digital library respectively.The medical case that meets the inclusion criteria is converted into text format,and the natural language processing technology is applied to standardize the process of data cleaning,word segmentation and synonym replacement through Python combined with Jieba phrase pieces.After manual verification and data filling,structured medical case information is formed.Taking feishu collaboration platform as database carrier,relevant functional modules are constructed and required data are uploaded to complete the construction of database.The use of ancient and modern basis cloud platform on the basis database in the basis of data mining,through mathematical statistics,correlation analysis,cluster analysis,methods of complex network analysis,on the basis of general information,clinical manifestation,card type,information analysis,such as the formulas to sum up the modern name old doctor of traditional Chinese medicine syndrome differentiation and treatment of chronic heart failure experience.Results1.In this study,a database of chronic heart failure medical cases of modern famous traditional Chinese medicine was finally constructed,which covers 137 cases and involves 71 modern famous traditional Chinese medicine doctors.It integrates diversified data query,authority management,communication and interaction,and is easy to log in and operate.2.Mining and analyzing 137 medical cases included in the database,the results are as follows:The patients with chronic heart failure included in this study were mainly in the age group of 50-79 years old,with the majority of patients in the age group of 60-69 years old(34.09% of the total).There were more male patients than female patients,with a male to female ratio of nearly2.2 to 1.The basic cardiac diseases mainly included coronary heart disease(37.50%),hypertensive heart disease(18.75%),rheumatic heart disease(16.07%),pulmonary heart disease(12.50%)and dilated cardiomyopathy(11.61%).Clinical manifestations,the most common symptoms of chronic heart failure patients are lower limb edema,chest tightness,less urine volume,shortness of breath,palpitations,shortness of breath,fatigue,unable to recumbent,poor appetite,cough;The most common tongue texture is purple dark,light dark,dark red,light fat,light red tongue;The most common coating on the tongue was thin white,white,white and greasy,little and thin yellow.The most common pulse symptoms are in order of sinking,counting,jie dai,chord and slippery.The core clinical manifestations of patients with chronic heart failure are edema of the lower extremities,poor urination,shortness of breath,shortness of breath,chest tightness,palpitations,fatigue,poor appetite,inability to lie flat,cough,cyanosis,dark purple tongue,white or thin moss,and heavy and thin pulse.A total of 12 syndromes were involved,with 5 main syndromes,including Yang deficiency,blood stasis and water stopping syndrome(24.82%),Qi deficiency,blood stasis and water stopping syndrome(16.06%),Yang deficiency and water flooding syndrome(12.41%),qi and Yin deficiency and blood stasis syndrome(10.95%),and phlegm-drink interaction syndrome(10.22%).In terms of treatment,the most commonly used prescriptions were Zhenwu Decoction,Ephedra Aconite Asarum Decoction,Shengmai Powder and Wuling Powder.The four qi of Traditional Chinese medicine are mostly warm,flat and slightly cold,the five tastes are mostly sweet,spicy and bitter,and the meridian of lung,spleen and heart is the main.The efficacy of the drug is mainly to improve temperature Yang,huoxue li shui;The most commonly used traditional Chinese medicine of poria cocos,astragalus membranaceus and salvia miltiorrhiza,atractylodes,monkshood,cassia twig,semen lepidii,rhizoma alismatis,radix ophiopogonis,liquorice;Commonly used medicines poria cocos respectively-atractylodes,poria cocos,salvia miltiorrhiza,atractylodes rhizoma alismatis,lateral root poria cocos,lateral root,salvia miltiorrhiza,salvia miltiorrhiza,astragalus root,astragalus monkshood,cassia twig tuckahoe,cassia twig atractylodes,alisma,poria cocos,semen lepidii astragalus,semen lepidii,tuckahoe,liquorice,poria cocos,dangshen,astragalus membranaceus,grifola poria cocos,dwarf lilyturf,fructus schisandrae,rhizoma ligustici wallichii salvia miltiorrhiza;For lower limb swelling commonly used poria cocos,atractylodes,salvia miltiorrhiza,lateral root atractylodis,Poria cocos,salvia miltiorrhiza,Astragalus membranaceus,Aconite seed,Osmanthus casei,Alisma,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine,Traditional Chinese medicine.Due to the large number of syndrome types involved and the small number of medical cases of some syndrome types,in order to better explore the law of syndrome differentiation,this study conducted dimension reduction analysis on syndrome types based on syndrome elements,retained the completeness of syndrome combination while taking into account the differences between groups,and divided the total database into Yang deficiency,Yin deficiency,Qi deficiency and phlegm-dampness sub-database for exploration respectively.Yang deficiency patients often can not be supine,fear of cold,cold limbs,lethargy,light tongue and other manifestations,body edema,urine volume and other symptoms of wet stop more serious.Treatment often reuse large doses of aconite with cassia twigs,often combined reuse poria cocos,Traditional Chinese medicine,Traditional Chinese medicine,such as leiocarpa,morosa leiocarpa,pigling water infiltration products.Core prescriptions are Monkshood root,Astragalus membranaceus,Poria cocos,Alisma laxma,Danshen,Morosa leizi,Osmanthus trunk,Atractylodes atractylodis.Yin deficiency patients often see dry mouth,night sweats,insomnia,constipation,red tongue,less moss,pulse and other manifestations,urine volume is often normal,the degree of edema is lighter.Treatment often reuse ophiopogon with jade bamboo,yellow essence and other nourishing Yin products,accompanied by schisandrae convergence solid astringent,convergence Yin shengjin,with a more gentle taste of radix pseudostellariae radix instead of codonopsis,ginseng,light dose of the use of water infiltration products to prevent excessive water and injury Yin liquid.The core prescriptions were Radix ophiopogonis,Astragalus membranaceus,Poria cocos,salvia miltiorrhiza,Atractylodes atractylodis,schisandra chinensis and glycyrrhiza.Qi-deficiency patients often have chest tightness as the main symptom and also chest pain,shortness of breath,weakness,and blood stasis symptoms are more severe,which is common in patients with heart failure based on coronary heart disease.In treatment,a large dose of astragalus membranaceus and Codonopsis pilosula are often used in combination to supplement and nourish zhongqi.In terms of water infiltration and dampness,atractylodes macrocephala and Codonopsis pilosula are mainly used to promote the metabolism of water dampness by invigorating qi and invigorating spleen.In terms of promoting blood circulation and removing stasis,a drug pair of Chuanxiong-Danshen is often used in combination with a small amount of Osmanthus cinnamomum.Core groups for Astragalus membranaceus,Atractylodes atractylodes,salvia miltiorrhiza,Poria cocos,Wheat winter,Traditional Chinese natural language processing technology on the basis of text data processing to enhance the basis database construction efficiency,collaboration platform as a basis to network database carrier is beneficial to the development of database application function,reduce the management and maintenance of artificial cost,the late the basis database construction method has certain enlightening significance.2.The empirical rule of the differentiation and treatment of chronic heart failure of modern famous traditional Chinese medicine can be summarized as follows:In syndrome differentiation,the deficiency of heart qi and Yang is the basis,and water,phlegm,turbid blood stasis is the standard.As the central link of the disease,blood stasis runs through the beginning and the end,which is the syndrome of deficiency of heart and deficiency of heart.In the specific classification,we should focus on the core symptoms and characteristic manifestations,clarify the degree of deficiency and standard reality,and combine disease differentiation of Western medicine with syndrome differentiation of Traditional Chinese medicine. |
