Objective:To observe the clinical efficacy of acupuncture combined with massage in the treatment of neck type cervical spondylosis under the guidance of the theory of ’treamemt of neck and waist together’,and to compare it with conventional acupuncture and massage therapy and oral western medicine in the treatment of neck type cervical spondylosis,in order to provide a new reference for clinical treatment of neck type cervical spondylosis.Methods:A total of 90 patients with neck type cervical spondylosis who met the diagnostic criteria were selected and divided into acupuncture and massage group(observation group),routine acupuncture and massage control group(routine group)and oral western medicine control group(western medicine group)by random number table method.The observation group was treated with acupuncture and massage on the neck and waist,once a day,6 times a week,one week as a course of treatment,a total of 2 courses of treatment.The routine group was treated with acupuncture and massage to treat the neck part,and the course of treatment was consistent with the observation group.The western medicine group was treated with oral diclofenac sodium sustained-release tablets(Futalin,Beijing Nuohua Pharmaceutical Co.,Ltd.,specification 75mg),once a day,1 tablet each time,1 week as a course of treatment,a total of 2 courses of treatment.After the end of treatment,SPSS26.0 statistical software was used to compare the VAS score,NDI score and total effective rate of the three groups before and after treatment,and the adverse reactions of the three groups were recorded.The VAS scores of the three groups were followed up one month after the end of treatment,and the clinical efficacy of the three groups was evaluated and analyzed.Results:(1)There was no significant difference in gender,age,course of disease,VAS score before treatment and NDI score before treatment(P>0.05)between the three groups,and could be compared.(2)VAS score:① Intra-group comparison:Compared with before treatment,the VAS score of the three groups was reduced(P<0.001),indicating that all three therapies could improve the neck pain of patients;② Intergroup comparison:After treatment,the VAS score of the observation group was significantly lower than that of the conventional group(P<0.05)and significantly lower than that of the western medicine group(P<0.05),while the VAS score of the conventional group was significantly lower than that of the western medicine group(P<0.05).It was suggested that in terms of relieving neck pain,the efficacy of the conventional group was better than that of the western medicine group,and the efficacy of the observation group was better than that of the conventional group and the western medicine group.(3)NDI score:①Intra-group comparison:NDI scores of the three groups decreased after treatment,and the interaction between different time nodes and treatment methods was significant(P<0.001),indicating that all three groups had efficacy;② Intergroup comparison:the NDI score of the observation group after treatment was significantly lower than that of the conventional treatment group(P<0.05),the NDI score of the observation group after treatment was significantly lower than that of the western medicine group(P<0.05),and the NDI score of the conventional group after treatment was significantly lower than that of the western medicine group(P<0.05),indicating that the observation group and the conventional group could better improve the neck dysfunction of patients than the western medicine group,but the observation group had better efficacy than the conventional group.(4)Comparison of curative effects:after the end of the course of treatment,6 cases were cured,19 cases were effective,4 cases were effective,and 1 case was ineffective,with a total effective rate of 96.67%;In the conventional treatment group,3 cases were cured,15 cases were effective,8 cases were effective,and 4 cases were ineffective,with a total effective rate of 90%,while in the western medicine group,1 case was cured,8 cases were effective,14 cases were effective,and 7 cases were ineffective,with a total effective rate of 76.67%.After the Ridit test,there was a significant difference in efficacy between the three groups(P<0.05).(5)Adverse reactions:During the treatment,one case of delayed needle occurred during treatment in treatment group 1,and the corresponding treatment was carried out,and the needle was removed immediately;In the conventional group,there was one manipulative reaction,manifested as neck pain and discomfort,which was relieved the next day;There were 2 gastrointestinal reactions in the western medicine group,manifested as mild abdominal distention,all of which were relieved within 1 day.There were no serious adverse reactions in Yu.Conclusion(s):(1)"Neck and waist treatment" acupuncture combined with massage,conventional acupuncture combined with massage,oral western medicine,these three therapies play different degrees of role in the improvement of neck pain and neck dysfunction,but the clinical efficacy of "neck and waist treatment" acupuncture combined with massage in the treatment of NTCS is better than that of conventional acupuncture massage and oral western medicine.(2)From the perspective of adjusting the overall mechanical balance of the spine,the theory of"neck and waist treatment" is used to treat cervical spondylosis,which is better than the local treatment of cervical spondylosis,and is worthy of clinical promotion and application.
