Objective: This study focuses on the treatment of chronic ulcer wounds in lower limbs using negative pressure sealing drainage technology. After the completion of negative pressure sealing drainage technology,it is supplemented with lightning fire moxibustion treatment in our hospital. The effectiveness and safety of this method are explored from multiple aspects, and its clinical application value is evaluated. Methods: According to the set inclusion and exclusion criteria,30 patients with chronic ulcer wounds in the lower limbs who were admitted to the Orthopedics Department of Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine from February 2022 to February2023 were selected. After inclusion in the study, the purpose and treatment plan of this study were explained to the patients and their families, and their consent was obtained.According to the different treatment methods of the patients, they were divided into a control group: VSD(n=15),and an observation group: Thunder Fire Moxibustion VSD(n=15)for treatment. After completing relevant examinations and eliminating surgical contraindications, negative pressure sealing drainage surgery was performed on all patients.Among them, the VSD group received symptomatic treatment of anti infection, swelling and pain relief after surgery. The patients in the Thunder Fire Moxibustion VSD group started using Thunder Fire Moxibustion from the first day after surgery until 14 days after surgery, once a day, each time moxibustion was performed on the Shenque and bilateral Zusanli acupoints. The rest of the routine treatment was the same as the control group.By comparing the classification count of white blood cells,C-reactive protein levels, erythrocyte sedimentation rate levels, growth factor indicators, chronic ulcer wound score, and VAS pain score before surgery,7days after surgery, and 14 days after surgery, the clinical efficacy of lightning fire moxibustion assisted negative pressure sealing drainage technology in the treatment of chronic ulcer wounds in the lower limbs was observed. Results: Compared with the baseline data of the two groups, the difference was not statistically significant(P>0.05)and was comparable.Comparison of different time points in WBC,CRP and ESR groups: WBC,CRP and ESR were gradually decreased on the 7th and 14 th day after operation compared with those before operation, and on the 14 th day after operation compared with those on the 7th day after operation, and the difference was statistically significant(P<0.05).Comparison between the two groups at the same time point: there was no statistically significant difference in WBC,CRP and ESR between the two groups before operation(P>0.05),but there was statistically significant difference in WBC,CRP and ESR between the two groups on the 7th and 14 th day after operation(P<0.05),and the improvement degree of the observation group was significantly greater than that of the control group, with a statistically significant difference(P<0.05).There was no significant difference in the concentration of b FGF and EGF in serum between the two groups before treatment(P>0.05);After treatment, the concentration levels of b FGF and EGF in the two groups of wound patients increased on the 14 th day,and the observation group was better than the control group, the difference was statistically significant(P<0.05).Comparison of the evaluation criteria of the curative effect of the two groups of patients after treatment showed that 15 patients in the observation group were effective, and the total effective rate was 100.00%;The total effective rate of the control group was 73.33%.Chi-square test showed that the curative effect of the observation group was significantly higher than that of the control group(P<0.05).There was no statistically significant difference in the wound score of chronic ulcer between the two groups before treatment(P>0.05).The wound area score of the two groups of patients 14 days after treatment was lower than that before treatment, and the wound score of the observation group was lower than that of the control group, the difference was statistically significant(P<0.05).Comparison of VAS pain score at different time points in the two groups: the VAS pain score on the 7th day after operation was compared with that before operation, and the VAS pain score on the 14 th day after operation was compared with that before operation and the 7th day after operation, the difference was statistically significant(P<0.05).Comparison between the two groups at the same time point: there was no statistically significant difference in VAS pain score before operation between the two groups, but there was statistically significant difference in VAS pain score at 7 and 14 days after operation(P<0.05).Conclusion(s):Thunder fire moxibustion assisted negative pressure sealing drainage technology can effectively reduce inflammatory indicators such as serum white blood cell count,C-reactive protein, and red blood cell sedimentation rate in the treatment of chronic ulcer wounds in the lower limbs, reduce inflammatory reactions, and help alleviate patient pain;At the same time,it can also promote the expression of b FGF and EGF in chronic wounds, stimulate the growth of granulation, and facilitate the healing of chronic ulcer wounds. The overall therapeutic effect is satisfactory, and it is worth promoting clinically.
