| Objective: By comparing simple VSD negative pressure drainage,this study observed the curative effect of Jiuhua cream combined with negative pressure drainage in the treatment of chronic skin ulcer with dampness and heat syndrome,and explored its possible mechanism.Methods: 72 patients with chronic skin ulcer who met the criteria and were included in the study were selected.The type of TCM syndrome differentiation is damp-heat syndrome.The patients were randomly divided into treatment group and control group,with 36 cases in each group.Patients in both groups received routine treatment;The control group received routine disinfection and dressing change,moderate removal of necrotic tissue on wound surface,VSD treatment,and replacement of negative pressure sponge and suction tube once every 2 days.On the basis of the treatment method in the control group,the treatment group was given Jiuhua cream once after 2 times of VSD treatment per line.The total course of treatment was 4weeks.On the 7th,14 th,21st and 28 th day of treatment,the reduction rate of wound area,the score of wound condition,the time of removal of necrotic tissue,the time of muscle generation and the situation of bacterial culture were recorded,and the efficacy was evaluated.Results: In the process of this study,5 cases were shed in the treatment group and 6cases in the control group,and 61 cases were finally completed.(1)The total significant efficiency of the treatment group was 51.613%.The significant efficiency of the control group was 12.667%,and the wound effective rate of the treatment group was significantly higher than that of the control group(P<0.05).(2)Comparison of wound area between groups: 1 and 2 weeks after treatment,the two groups were similar(P>0.05),3 and 4 weeks after treatment,the treatment group was significantly higher than the control group(P<0.05);Comparison within groups:the area of the two groups was not significantly reduced at the first week(P>0.05),but significantly decreased at the second,third and fourth week(P<0.05).(3)The comparison of the overall evaluation of wound surface between groups showed that there was no significant difference between the two groups in the first week of treatment(P>0.05),and the curative effect was similar.After the second,third and fourth week,the treatment group was better than the control group(P<0.05).Comparison of scores: there were significant differences in wound depth and surrounding skin color between the treatment group and the control group(P<0.05),and the situation of the treatment group was better than that of the control group.There were no significant differences in marginal epidermis,necrotic tissue type,exudate type,exudate volume and granulation tissue(P>0.05);After 4 weeks of treatment,the wound surface of the two groups was significantly improved(P<0.05).(4)The removal time of necrotic tissue was significantly different(P<0.05).There was significant difference in the time of muscle generation(P<0.05),and the treatment group was earlier than the control group.(5)Comparison of bacterial culture results between the two groups: there was no significant difference between the two groups(P>0.05).Intra-group comparison: the detection rate of both groups decreased significantly after 7 days of treatment(P<0.05).(6)Adverse reactions: 2 patients in the treatment group had pain,and 1 patient in the control group had pain,both caused by the stiffening of the sponge and the stimulation of the wound.2 Patients in the control group had pruritus,which was caused by allergy caused by the negative pressure film.(7)Safety: the two groups were the same without serious adverse reactions.Conclusion: Jiuhua cream combined with VSD negative pressure in the treatment of Damp-heat syndrome chronic skin ulcer can effectively promote the repair of the wound surface,and it is not easy to appear toxic and side effects and adverse reactions... |
