Objective:This study aims to figure out the correlation between nNO concentration and CRS and analyze the correlation between nNO concentration and visual analog scale(VAS)score,Lund-Kennedy score,and Lund-Mackay score in patients with CRS by studying the changes in nasal exhaled nitric oxide(nNO)concentration in patients with chronic rhino sinusitis in Dalian area.It also explores whether nNO concentration detection could provide a basis for non-invasive diagnosis of CRS and estimates its value in clinical practice.Methods:Sixty patients with CRS who attended the second hospital of Dalian Medical University from May 2022 to February 2023 were selected(case group),and 50 healthy volunteers were recruited(control group)for a total of 110 subjects.The inclusion criteria for the case group were patients with CRS aged between 18 and 70 years old;no other inflammatory diseases of the upper and lower respiratory tract,no history of medication of related illnesses,no consumption of nitrogen-rich foods such as spinach within 2 hours before the test;no consumption of caffeine-rich beverages;no strenuous exercise within 2 hours before the test;no alcohol consumption within 4 hours before the test;and the subjects were able to cooperate with all tests fully.Inclusion criteria for the control group: no history of acute or chronic rhinitis or sinusitis,no history of systemic or local allergic diseases,no obvious nasal discomfort,no abnormal nasal signs on physical examination,and regular blood test results.General data were collected on all included subjects,VAS scores were performed on subjects in the case group,Lund-Kennedy scores were calculated for the case group based on nasal endoscopy,and Lund-Mackay scores were calculated based on sinus CT images.nNO concentrations were measured in all subjects who met the inclusion criteria,and the tests were performed using a naculon breath analyzer Sunvou-CA2122(Wuxi,China).SPSS 23.0statistical software was applied to analyze the data,and for data that met normal distribution,an independent sample t-test was selected for comparison.Logistic regression coefficient models were constructed to analyze whether nNO concentration influenced the prevalence of chronic sinusitis.Correlations between nNO concentrations and VAS scores,LundKennedy scores,and Lund-Mackay scores were assessed using Pearson.The receiver operating characteristic(ROC)curve,referred to as the ROC curve,was plotted to analyze the predictive diagnostic value of nNO concentration for chronic sinusitis.nNO concentration was said to be highly predictive of chronic sinusitis when the area under the curve(AUC)> 0.9,0.7 < AUC ≤ 0.9 showed moderate predictiveness,0.5 < AUC ≤ 0.7showed low predictiveness,but when AUC = 0.5 was considered to have no predictive value.The cut point also clarified the best critical matter with the most extensive Jorden index(sensitivity + specificity-1),and all results were considered statistically significant at P < 0.05.Results:1.nNO concentration levels were significantly lower in CRS patients than in healthy subjects(p<0.01).2.The ROC curve showed AUC=0.935,p<0.01.The maximum value of the Jorden index for nNO concentration was 0.767,and the optimal threshold value was 612 ppb.3.nNO concentration showed a significant negative correlation with VAS score(r=-0.482,P<0.001);nNO concentration showed a significant negative correlation with Lund-Mackay score(r=-0.416,P<0.001);nNO concentration showed a significant negative correlation with Lund-Kennedy score(r=-0.402,P<0.001)Conclusions:1.nNO concentration can better distinguish patients with chronic sinusitis from healthy people and has high sensitivity and specificity in diagnosing chronic sinusitis.nNO concentration measurement is valuable as a quick and non-invasive test for the diagnosis of chronic sinusitis.2.nNO concentration correlated with VAS score,Lund-Kennedy score,and Lund-Mackay score of patients with chronic sinusitis,suggesting that nNO concentration correlates with the severity of the disease.
