| Objective: To observe the clinical efficacy and safety of Huanglian Wendan Decoction combined with acupoint application in the treatment of phlegm-heat disturbed heart type insomnia after stroke,so as to improve the sleep quality of patients with insomnia after stroke and improve the ability of daily living activities through the combination of internal and external treatment of traditional Chinese medicine,and explore the auxiliary effect of infrared thermoimage as a clinical observation index of traditional Chinese medicine to evaluate the efficacy.To provide a new solution and objective basis for the clinical treatment of post-stroke insomnia.Methods: The patients in this study were recruited from the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from November 2021 to November 2022,and 64 patients who were eligible for diagnosis of post-stroke insomnia caused by phlegm-heat disturbance were screened out.By using random number table method,the patients were randomly divided into the experimental group and the control group,32 cases in each group.Patients in both groups were given conventional treatment for stroke,the control group was given oral Huanglian Wendan decoction on this basis,the experimental group was given acupoint application on the basis of the control group,and the points were selected on both sides: Shenmen,Neiguan,Sanyinjiao,Zhigou and Fenglong.Chinese medicine was warm taken twice in the morning and evening,and applied once a day for 6 hours,5 times a week.Both groups were treated continuously for 2 weeks.The study data were analyzed using SPSS26.0,and the changes of TCM syndrome score,Pittsburgh sleep quality index score,activity of daily living score and infrared calorific value before and after treatment were compared,and the efficacy evaluation was conducted between the two groups.At the same time,the occurrence of adverse events during treatment was observed to evaluate the safety of treatment in the two groups.Results:1.General data analysis: 64 patients were included in this study,32 in the test group and 32 in the control group,4 cases were eliminated or dropped,and 60 cases were finally completed.There was no significant difference in gender,age and stroke type between the two groups(P > 0.05),indicating comparability.2.Efficacy evaluation: TCM efficacy was evaluated according to the score reduction rate of TCM syndrome score evaluation table.The total effective rate of experimental group was 93.33% and that of control group was 86.67%,which was significantly better in experimental group than control group(P < 0.05).According to the Pittsburgh Sleep Quality Index(PSQI)reduction rate,the total effective rate of the experimental group was 93.33% and the total effective rate of the control group was 83.33%,which was significantly better in the experimental group than in the control group(P < 0.05).3.Scale score comparison: There were significant differences in TCM syndrome score and Pittsburgh sleep quality index score between the two groups before and after treatment(P < 0.01),and there were significant differences in Barthel scale score between the two groups(P < 0.05).The three scores after treatment were improved compared with those before treatment;After treatment,there were significant differences in scores between the groups,among which TCM syndrome score and Pittsburgh sleep quality index score in the test group were significantly lower than those in the control group(P < 0.01),and Barthel score in the test group was higher than that in the control group(P < 0.05).4.Infrared calorific value comparison: There were significant differences in the observation area of infrared calorific value between the two groups before and after treatment(P < 0.01 in the experimental group and P < 0.05 in the control group),and the infrared calorific value of frontal area and Shanzhong point decreased significantly compared with that before treatment.Moreover,there were also significant differences in the comparison of infrared calorific value of frontal area and shanzhong point between the two groups after treatment(P < 0.05),and the temperature of the experimental group was significantly lower than that of the control group.5.Safety assessment: No adverse events occurred during treatment,and the safety was high.Conclusion:1.Huanglian Wendan Decoction combined with acupoint application has obvious therapeutic effect on insomnia patients with phlegm-heat disturbed heart type after stroke,which is superior to simple Chinese medicine treatment,can effectively improve patients’ sleep quality,activities of daily living and traditional Chinese medicine syndrome,regulate body surface heat value,and has no adverse reactions,and is worthy of application.2.Infrared thermography can be used as a visual diagnostic tool to assist clinical efficacy evaluation. |
