| Objective: To investigate the clinical effect of Xingqi Huoxue Formula recipe on hypoparathyroidism after total thyroidectomy.Methods:60 patients with thyroid cancer undergoing total thyroidectomy in Affiliated Hospital of Shandong University of Chinese Medicine were selected and randomly divided into experimental group(30 cases)and control group(30 cases).They were treated with routine calcium supplement or combined with Xingqi Huoxue Formula recipe(30 cases).The indexes of blood PTH,blood Ga,drainage fluid PTH and TCM syndrome score before and after treatment were detected at different time points of the two groups.Results:(1)There was no significant difference in age,gender and number of surgical methods between the two groups(P>0.05),indicating comparability.(2)The mean blood PTH levels of the experimental group and the control group on the first day after surgery were 14.85±2.85pg/ml and 15.70±2.15pg/ml,respectively;The mean blood Ca levels were 1.96±0.19mmol/L and 1.72±0.20mmol/L,respectively.The detection indexes of both groups on the first day after surgery were lower than the preoperative value or normal value,and the absolute value of the difference between them and the preoperative value was: Blood PTH levels were 31.06±8.11 and 29.84±9.80pg/ml,respectively,and blood Ca levels were 0.44±0.05 and 0.74±0.09mmol/L,respectively,and there was no statistical significance between the two groups(P>0.05),indicating that patients had indicators or symptoms of hypoparathyroidism.(3)Intra-group comparison: the blood PTH and Ca recovery levels of the experimental group and the control group on day 1,day 3,day 5 and day7 after surgery were gradually increased,and the mean blood PTH levels of the experimental group on day 1 and day 7 after surgery were 14.85±2.89 and 54.81±11.94pg/ml,respectively.The mean blood Ca levels were1.96 ± 0.19 and 2.56 ± 0.10mmol/L,respectively.The difference was statistically significant(P<0.05).The mean blood PTH level of the control group was 15.70±2.15 and 42.55±8.54pg/ml on day 1 and day 7 after surgery,respectively.The mean blood Ca levels were 1.72±0.20 and 2.34± 0.10mmol/L,respectively,and the differences were statistically significant(P<0.05).According to the experimental results,there were statistically significant differences between the two groups on the 1st day after surgery and the 3rd and 5th day after surgery(P<0.05).The level of PTH in the drainage fluid of the experimental group and the control group on day 1,day 3 and day 5 after surgery was gradually decreased,and the mean level of the drainage fluid of the experimental group on day1 and day 5 after surgery was 2023.1±192.5 and 875.7±300.8pg/ml,respectively.The difference was statistically significant(P<0.05).The mean level of PTH in the drainage fluid of the control group on the 1st and 5th day after surgery was as follows: 1990.3 ± 186.0,1013.1 ±179.8pg/ml,the difference was statistically significant(P<0.05),according to the experimental results,the difference between the two groups on the first and third day after surgery was statistically significant(P<0.05).It can be concluded that after treatment,the recovery levels of blood PTH and blood Ca in the two groups gradually increased,while the PTH level of drainage fluid gradually decreased,indicating that both routine calcium supplementation and combined treatment with Chinese herbal medicine of Xingqi Huoxue Formula can promote the recovery of postoperative parathyroid gland and achieve therapeutic effect.(4)Comparison between the two groups: According to the experimental results,the recovery level of blood PTH and blood Ca and the decrease level of drainage fluid PTH in the experimental group were significantly better than those in the control group,and the difference between the two groups was statistically significant(P<0.05),indicating that the combination of Xingqi Huoxue Formula treatment on the basis of conventional treatment can further promote the recovery of parathyroid gland.The effect of prevention and treatment of hypoparathyroidism and hypocalcemia is more significant.(5)Total thyroidectomy of 60 enrolled patients was divided into 3 types.Among the same operation methods,they could be divided into experimental group and control group according to routine calcium supplement therapy and combined Xingqi Huoxue Formula therapy.After observing the blood Ca level at various time points after surgery,it was found that between the two groups: The postoperative blood calcium recovery level of patients in the experimental group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).By comparing the blood Ca level at different time nodes of different operation methods in the experimental group,it can be concluded that Xingqi Huoxue Formula can also improve the blood Ca level between different operation methods.In addition,according to the range of different operation methods from small to large,it can be concluded: The absolute values of the difference in blood Ca levels on day 1 and day 7 after surgery were 0.66±0.09,0.56±0.13,and 0.39±0.09mmol/L,respectively,indicating that the degree of increase in blood Ca was affected by the scope of surgery.The effect of Xingqi Huoxue Formula on thyroid cancer patients with a smaller scope of total resection was more significant.(6)Comparing the scores of TCM symptoms between the two groups,the difference values of the experimental group on the 1st and 7th day after surgery were 12.25±2.10,6.75±2.86,5.5±3.14,respectively;The total efficiency is 83.3%.The control group was 14.70 ± 2.23,4.95 ± 2.87,7.75 ± 2.77,respectively.The total efficiency is 56.7%.The overall recovery effect of the experimental group was significantly better than that of the control group,and the difference between the two groups was statistically significant(P<0.05),indicating that the oral administration of Xingqi Huoxue Formula could promote the recovery of postoperative parathyroid function injury of patients in the experimental group.Effective improvement of postoperative shortness of breath,lazy speech,pale face,numbness of limbs,neck pain,refusal to press after the syndrome of qi stagnation and blood stasis symptoms,limbs,mouth numbness,pain,hand and foot convulsions para-thyrothyroidism or hypocalcemia symptoms.Conclusion: The combination of Xingqi Huoxue Formula formula on the basis of routine calcium supplementation can promote the recovery of parathyroid gland after operation,improve the function of parathyroid gland,and effectively improve the postoperative symptoms of shortness of breath and lazy speech,pale face,numbness of limbs,neck pain,and refusal to press the postoperative symptoms of qi stagnation and blood stasis,numbness and pain around the limbs and mouth,hand and foot convulsion para-thyroid reduction or hypocalcemia.Secondly,Xingqi Huoxue Formula decoction can also increase serum PTH and blood Ca levels and improve postoperative parathyroid function between different total thyroidectomy operations for thyroid cancer,and the efficacy of patients with small surgical scope is better than that of patients with large surgical scope. |
