Objective: To observe the clinical effect of the sequential therapy of traditional Chinese medicine in the treatment of bronchial asthma patients with acute attack of cold asthma and its influence on intestinal flora,and to explore the microbiological effect mechanism of the sequential therapy of traditional Chinese medicine in the treatment of asthma.Methods:In this study,60 patients with asthma who met the diagnostic criteria and inclusion criteria were randomly selected and divided into treatment group and control group by random number table method,with 30 patients in each group.The course of treatment was 2 weeks.The control group was treated with the standard Western medicine,and the treatment group was treated with the sequential traditional Chinese medicine prescription(Linggui Zhugan decoction + Baitong decoction)on the basis of the standard Western medicine.The clinical efficacy,ACT score,lung function and intestinal flora of the two groups were observed before and after treatment.In addition,30 healthy people were collected as the healthy group to observe the changes of intestinal flora,and jointly analyze the clinical effect of traditional Chinese medicine prescription sequential therapy on patients with cold asthma and the mechanism of intestinal flora changes.Results:1.After 2 weeks of treatment,the total effective rate of the treatment group was 92.9%,the total effective rate of the control group was75.9%,the total effective rate of the treatment group was higher than that of the control group,and the clinical effect of the treatment group was better than that of the control group,the difference was statistically significant(P<0.05).The total effective rate of the treatment group was 92.9%,and that of the control group was 72.4%.The improvement of the curative effect of the treatment group was better than that of the control group,and the difference was statistically significant(P<0.05).The improvement rates of FEV1,FEV1/FVC% and FEV1 were increased in treatment group and control group after2 weeks.The improvement effect of treatment group was better than control group(P<0.05).After treatment,ACT scores of the two groups were significantly increased,and the treatment group was significantly better than the control group,the difference was statistically significant(P<0.05).Ig E levels in both groups were lower than before treatment,and the treatment group was better than the control group,the difference was statistically significant(P<0.05).2.Intestinal flora sequencing results: the total number of OTUs was 2223,and the total number of OTUs in the five groups was 112.The number of OTUs in each group was 1286 in the healthy group,764 before treatment,1026 after treatment,715 before treatment,and 1135 after treatment.Analysis of bacterial community structure: at the phylum level,the top three groups were Bacteroidetes,Firmicutes and Proteobacteria,accounting for more than 90%of each group.At the genus level,compared with the control group,the abundance value of bifidobacterium in the treatment group increased significantly after 2 weeks of treatment,which was closer to that of the healthy group.Bifidobacterium is beneficial bacteria.Alpha diversity analysis showed that after 2 weeks of treatment,Chao1 index and Shanon index of control group and treatment group increased significantly compared with before treatment,and the increase of treatment group was more obvious than control group,and the difference between the two groups was statistically significant(P<0.05).Chao1 index and Shanon index increased after treatment in the treatment group,indicating that the richer and more diverse intestinal microbial species were.Bate diversity analysis showed that after 2 weeks of treatment,the intestinal flora structure of the treatment group was more similar to that of the healthy group,indicating that the healthier the intestinal flora structure was,the more conducive to intestinal homeostasis.Conclusion: 1.The sequential therapy of traditional Chinese medicine can improve the clinical symptoms and lung function of patients with cold asthma in the acute stage of bronchial asthma.2.The sequential therapy of TCM prescription can regulate the richness and diversity of intestinal flora in patients with asthma and restore the homeostasis of intestinal microbes.3.There may be an immune regulation relationship between intestinal flora and Ig E in asthmatic patients,which may be the therapeutic mechanism of TCM sequential therapy intervention in asthmatic patients.
