Objective:This study analyzed the clinical efficacy of acupoint embedding combined with metformin in the treatment of infertility caused by phlegm-damp polycystic ovary syndrome to provide new ideas and strategies for the clinical treatment of patients with infertility caused by phlegm-damp polycystic ovary syndrome,to improve the sensitivity to western drugs for ovulation and to improve the clinical pregnancy rate.Methods:From January 2021 to June 2022,83 patients(aged 20-35 years)with infertility caused by phlegm-wet polycystic ovary syndrome who met the standard and were collected from the Department of Reproduction and Genetics of Shandong Provincial Hospital of Traditional Chinese Medicine were randomly divided into experimental group(Acupoint embedding combined with metformin group)and control group(Metformin group).There were 41 cases in the experimental group and 42 cases in the control group.Among them,the control group was treated with metformin,with 4 weeks as a course of treatment and 3 courses of treatment.On the basis of the control group,the experimental group added acupuncture point embedding at acupoints for 10 days during non-menstrual period(Day 2 after menstruation)as a period.The acupoints selected were Zhongji,Guanyuan,Sanyinjiao,Zusanli,Fenglong,Zigong.The treatment was twice a month,and one course of treatment was one month,a total of three courses of treatment.During the treatment,both the control group and the observation group were orally administered Diane-35 for 3 months at the same time to adjust the menstrual cycle.After 3 months,all patients were treated with letrozole to promote ovulation and continuously observed for 3 months.The subjects in both groups collected basic information such as age,measured BMI and waist circumference,checked sex hormones,fasting blood glucose,fasting insulin and lipid-related indexes before and after receiving treatment,and performed the evaluation of the Chinese medicine evidence score quantification scale,and monitored the cycle ovulation rate and pregnancy rate.To determine whether there is a difference in the clinical outcome of the two treatments.Results:(1)After treatment,the number of ovulation in the experimental group was 21(52.5%)and 14(35%)in the control group;the number of pregnancy in the experimental group was 9(22.5%)and 6(15%)in the control group;both groups showed a greater degree of improvement after treatment,with statistically significant differences(P<0.05),and the ovulation and pregnancy rates in the experimental group were higher than those in the control group by chi-square test.(2)Before treatment,there was no statistical difference between the experimental group and the control group in terms of TCM syndrome points;after a period of treatment in both groups,there was a significant improvement in the TCM syndrome points;there was a statistically significant difference between the experimental group and the control group in terms of TCM syndrome points(P<0.01),and the experimental group had a significantly better treatment effect than the control group.(3)LH,T,and LH/FSH levels were reduced in both the experimental and control groups after treatment,with statistical differences before and after treatment(P<0.05);from the comparison between groups,LH was reduced more significantly in the experimental group than in the control group after treatment,with statistically significant differences(P<0.01).(4)Waist circumference,weight,and BMI were statistically different between the experimental and control groups before and after treatment(P<0.05);patients in the experimental group had better waist circumference,weight,and BMI reduction than the control group(P<0.05).(5)From the aspect of lipid-related indexes,there were statistical differences between the two groups in TG,TC,HDL and LDL before and after treatment(P<0.05),indicating that both treatments were effective;there were no statistical differences between the control and experimental groups in TC,TG and HDL after treatment(P>0.05);the experimental group was better than the control group in LDL(P<0.05).(6)From the viewpoint of insulin resistance-related indexes,there was no statistical difference between the insulin resistance-related indexes of the two groups before treatment(P>0.05);the improvement effect of several indexes such as FBG and HOMA-IR after treatment was better in the experimental group than in the control group(P<0.05),indicating that the experimental group had better treatment effect in insulin resistance.(7)The vital signs of patients in both groups were stable during treatment,and no allergic reactions and serious adverse reactions were reported.Individual subjects in the treatment group mentioned slight pain at the acupoint catgut embedding site,but no infection.In terms of adverse drug reactions,patients in both groups had mild nausea,diarrhea and other gastrointestinal reactions before taking metformin.Conclusion:Acupoint embedding combined with metformin has a good effect on improving ovulation rate,pregnancy rate and TCM syndrome of infertility patients caused by phlegm-wet polycystic ovary syndrome,which is better than oral metformin alone.It is worth further research and clinical promotion.
