| Objective:This project evaluates the therapeutic effect of Sheng Yu Prescription in the treatment of ulcerative colitis through clinical observation,and explores the modulating effect of Sheng Yu Prescription on the Nrf2-ARE pathway using animal experiments.This study will lay the foundation for further investigation of the mechanism of Sheng Yu Prescription in the treatment of ulcerative colitis from the perspective of anti-oxidative stress.Methods:1.Clinical study:Sixty-four patients with mild to moderate ulcerative colitis who met the inclusion criteria were divided into treatment and control groups according to the random number table method.The treatment group was given Sheng Yu Prescription and the control group was treated with mesalazine.The modified Mayo score,the efficacy of TCM symptoms and the incidence of adverse reactions in each group were recorded,and the data were statistically analysed at the end of the treatment cycle to evaluate the effectiveness and safety of drug treatment in both groups.2.Experimental study:Forty-four SPF mice were selected and divided into blank group,model group(DSS group),Sheng Yu Prescription treatment group(DSS+SY group)and mesalazine treatment group(DSS+MS group)using the random number table method.The model group was treated with saline gavage,the Sheng Yu Prescription group with Sheng Yu Prescription gavage and the Mesalazine group with Mesalazine gavage.After 28 days of treatment,the mice were observed for macroscopic changes and the disease activity index(DAI)was calculated;the expression of Nrf2 and HO-1 mRNA in colonic tissues was also measured by RT-PCR.Results:1.Clinical study:The results of the study showed that there was a statistically significant difference between the treatment group and the control group in all indicators before and after treatment(p<0.001),indicating that both groups had significant efficacy,but the difference between the groups in terms of clinical efficiency,clinical remission rate,and modified Mayo score was not statistically significant(P>0.05),and the efficacy was comparable.In terms of improvement in the efficacy of TCM symptoms,the difference between the groups was statistically significant(P<0.05)and it can be concluded that the treatment group was better than the control group in terms of improvement in TCM symptoms.There were no adverse reactions during the treatment period in both groups,and the short-term recurrence rate was not statistically significant(P>0.05)when compared between the groups at the 3-month follow-up.2.Experimental study:(1)Disease activity index(DAI)scores:after the end of treatment,the DAI scores of mice in the DSS+SY and DSS+MS groups were improved to some extent compared with the model group(P<0.01).(2)The mRNA expression of HO-1 and Nrf2 was significantly higher in the other groups compared with the blank group(P<0.01),and the mRNA expression of HO-1 and Nrf2 was significantly higher in the DSS+SY and DSS+MS groups compared with the model group(P<0.01).Compared with the DSS+MS group,the difference in mRNA expression of HO-1 in the DSS+SY group was not significant(P>0.05),but the difference in mRNA expression of Nrf2 in the DSS+SY group compared with the DSS+MS group was statistically significant(P<0.05),and it can be concluded that the mRNA expression of Nrf2 in the DSS+SY group was superior to that in the DSS+MS group.Conclusion:1.The clinical efficacy of Sheng Yu Prescription in treating patients with ulcerative colitis(large intestine damp heat type,E1,E2,mild-moderate)is remarkable,with outstanding effects in improving patients’ Chinese medical symptoms,while not significantly different from the effects of single western drug treatment,and the drug has high safety and low recurrence rate,which is conducive to maintaining long-term clinical remission.2.Animal experiments and RT-PCR assays showed that Sheng Yu Prescription can regulate the Nrf2ARE signaling pathway,upregulate the expression of Nrf2 and HO-1 mRNA in colonic tissues,enhance tissue resistance to oxidative stress and promote intestinal mucosal barrier repair. |
