Objective:To observe the efficacy and safety of acupuncture combined with invigorating spleen and tonifying lung in the treatment of chronic obstructive pulmonary disease(lung and spleen deficiency)with sleep disorder,and to improve the clinical symptoms of patients and improve the quality of sleep and life through the combination of internal and external treatment of traditional Chinese medicine,to provide a reference for clinical treatment of the disease.Methods:Sixty patients with chronic obstructive pulmonary disease(lung and spleen deficiency)with sleep disorder were selected and randomly divided into control group and treatment group according to the simple numerical table method,with 30 patients in each group.Both groups were treated with conventional western medicine and insomnia health education as the basic treatment,while the treatment group was treated with acupuncture(Shenmen,Taiyuan,Feishu,Pishu,Zusanli,Sanyinjiao,Zhaohai and Shenmai)combined with invigorating spleen and tonifying lung(Shenling Baizhu Powder)on the basic treatment,and the therapeutic effect was observed after 2 weeks.The TCM syndrome score,6MWT,arterial gas analysis index,CAT score,and PSQI score were recorded for both groups before and after treatment,and the data were processed and analyzed by SPSS26.0 software to evaluate the treatment effect and draw conclusions for both groups.Results:1.A total of 60 patients with chronic obstructive pulmonary disease(Qi deficiency of lung and spleen)with sleep disorder were included,including30 patients in the control group and 30 patients in the treatment group.Before treatment,there was no statistical difference in age,sex,course of disease,TCM symptom score,6MWT,arterial blood gas analysis,CAT score and PSQI score between the two groups(P>0.05),which was comparable.2.The total effective rate was 76.67% in the control group and 93.33% in the treatment group,and the curative effect in the treatment group was better than that in the control group(P<0.05).The scores of TCM symptoms in the two groups were significantly improved after treatment(P<0.01),indicating that both groups are effective in improving TCM symptom scores.There was no statistical difference between the two groups except spontaneous sweating and fatigue(P>0.05),but the treatment group had more advantages in cough,expectation,wheezing,difficulty sleeping,shallow sleep,anorexia,fatigue and total score(P<0.05).3.After treatment,the 6-minute walking distance between the two groups was improved(P<0.01),and the 6-minute walking distance improvement level in the treatment group was better than that in the control group(P<0.05).4.After treatment,the levels of Pa O2 and Pa CO2 in both groups were significantly improved(P<0.01).And the improvement degree of the treatment group was better than that of the control group(P<0.05).5.After treatment,the CAT scores of both groups were lower than before(P<0.01);compared with the control group,the improvement of CAT score in the treatment group was more obvious(P<0.05).This suggests that both groups are effective,with the treatment group being more effective.6.After treatment,the scores of all factors and total scores of PSQI in the two groups were lower than before(P<0.01),and it was found that the scores of all factors and total scores in the treatment group were lower than those in the control group(P<0.05),especially in the quality of sleep and daytime function,and the curative effect of the treatment group was significantly better than that of the control group(P<0.01).7.There were no significant adverse reactions during treatment in either group and the application was relatively safe.Conclusion:Acupuncture combined with the method of invigorating spleen and tonifying lung has a significant clinical effect on chronic obstructive pulmonary disease(Qi deficiency of lung and spleen)with sleep disorder,which is superior to the simple routine treatment of western medicine.It can reduce TCM symptom score,CAT score and PSQI score,improve arterial gas analysis index and improve walking distance by 6 minutes,and it is safe.No significant adverse reactions and side effects were observed;it is worth promoting in clinics.
